SCHOOL BUS DRIVER

CONSENT FORM
In accordance with Manitoba Regulation 465/88R, the Pupil Transportation Unit
(PTU), Manitoba Education issues School Bus Operator’s Certificates to all qualified
school bus drivers. PTU maintains a registry of all certified school bus operators in the
province. School bus driver licence information may be required by PTU to ensure
compliance with certification requirements.
In order for PTU to obtain personal driver’s licence information from Driver and
Vehicle Licensing, Manitoba Public Insurance, each school bus driver must give
consent by completing and signing this form.

I hereby authorize Manitoba Public Insurance and, in particular, Driver and Vehicle
Licencing, to disclose personal information regarding my Manitoba driver’s licence to
the Pupil Transportation Unit, Manitoba Education. The specific information I authorize
to be disclosed includes my name, address, date of birth, driver's licence number,
effective date, expiry date, endorsements, conditions (including expiry dates),
restrictions, dates of changes in licence status, reasons for changes in licence status,
and any other personal information reasonably required by the Pupil Transportation Unit
to enable it to administer school bus operator certification provisions under The Public
Schools Act (Manitoba) and Regulations.

This personal information is being collected under authority of The Public Schools Act, M.R. 465/88R, and will be used for ongoing verification of
certification and notification. It is protected by the Protection of Privacy provisions of The Freedom of Information and Protection of Privacy Act.
