We wish to introduce Ms. ANGIELYN TORCENDE of our company Mindshare Multipurpose

Cooperative for our client Richprime Sales Inc as Sales Promodiser. She will be responsible in
selling, promoting and refilling the product of the client.

In her stay with Gaisano Grand Group of Companies, Ms. TORCENDE is directly employed by
Mindshare Multipurpose Cooperative who shall be responsible for her salaries/wages/benefits as
provided for by law. She will under our control and supervision and shall not perform any duties of
your regular employees.

MS. TORCENDE will be assigned to one of your outlets/branches at Gaisano Grand Fiesta Mall
starting December 3, 2021 – March 3, 2022. She will abide/follow the rules of our Agency which
will align with the general rules and regulations set forth by your company.

Likewise, in cases where she commits/violates any of the Company rules of Gaisano Grand Group of
Companies, you as our client, may coordinate with us, so we can take action and impose proper
disciplinary action. While she is being investigated, we guarantee to provide a replacement for her. In
case of pilferage and other crimes that may result to damage to your company and if proven guilty
during investigation, our company has the sole responsibility to return and payoff/charge the loss
incurred as a result of the pilferage investigation and/or to any similar incidents such as mentioned
above.

Her monthly quota set from her is P_________/month.

Duty days: SKELETAL Day-off: Thursday
Schedule of Meeting: Upon visit of AM Time Schedule: 10:30am to 9:00pm
Break time: 3:00 – 4:00 Duration of months: (3) Months

violation committed by our employee.
