Directions in filling-out the Reading Progress Monitoring Chart and Numeracy Progress Monitoring Chart:
1. Check the focus competency/task for the set of learners.
2. List the learners under each competency/task.
3. Use the color spectrum in reporting the reading level/progress of each learner.
4. When the learner achieves the “green category,” he/she shall advance to the next competency until all the competencies are mastered.
5. At the end of each quarter, the teacher shall submit the summary of learners’ progress to the school head for consolidation.
