I, Mrs. Usha P authorised person of “TANN UTILITY SERVICES PVT, LTD” having its
registered office at #611, ‘C’ Block, ACES layout, Kundalahalli, Bengaluru – 560037,
here by we are declare and file this indemnity bond.
We would like to inform you that, Mrs. SHOBHA bearing EPF member account
No. PYKRP00352470000012317 and UAN No: 100352903984, was updated her
Name, Husband Name, Date of Birth and Relation of member i.e., SHOBA D/o
HANUMANNA, DOB: 12-08-1965 instead of Mrs. SHOBHA W/o VIJAYA KUMAR and
DOB: 08-12-1968, wrong updation was happened while in the registration of EPF
account number in the PF portal. This was happened due to some typing error while
in the registration of employee personal details.
Here by we are declaring our employee by the name of Mrs. SHOBHA, In PF records
her details updated as Ms. SHOBA D/o HANUMANNA & DOB: 12-08-1965 which is
not belonging or matching with any of TANN UTILITY SERVICES PVT LTD, records or
data. In case any of the duplications we are solely responsible for the same.
Therefore, request you to kindly make necessary corrections against
Mrs. SHOBHA, PF Account no PYKRP00352470000012317 and UAN No:
100352903984, Changes of her Name, husband name, Date of birth and Relation of
member we are hereby undertaking and Indemnity Central board of Trustees,
employee provident Fund organisation, Ministry of Labour, Govt of India, that this
changes if later found falls, we shall be responsible for any incurred out of this
transaction.
We further undertake that any liability arises short of this rectification.
Necessary form duly filled in is attached herewith.
