(CHANGE THE CURRENT 2.0GRANT CONTRACT AGREEMENT “This grant contract agreement is between Philippine Seatizens In. ("SPS") , Jessie “Mamites,Beejie Sheen Tugot, Nitta Jessa Jaralve, and Relyn Obnimaga.("GRANTEE"). Purpose of the Grant ‘SPS is providing grant funding for Eligible Expenditure incurred by the Grantee to implement the project entitled: Local Creek Conservation Network for Cimate Change ‘Adaptation in Sibutad, Zamboanga del Norte (“the Project”). The Project outputs and activities are set out inthe revised Project Proposal (Link). ‘The Grantee accepts responsibility forthe proper use and administration of all finding provided under this Grant Agreement and undertakes to use such funding only for the purpose of carrying out the Project in accordance with the Project Proposal. Grant Funding Period ‘The grant funding period is from May 1, 2023 to August 31, 2023. Amount of the SPS will provide up o a maximum of P14,950 towards the total costs of the Project. ‘Timing of Grant payments Payment will be made in two tranches. The first of which will be 50% of the gran, [7.475], released upon submission and approval of the following documents: Signed and retumed a copy ofthis Gramt Agreement to SPS; 1. Provided appropriate benk details or fund transfer channels; 2. Updated proposal and budget; and 3. Agreed to comply with the terms ofthis Grant Agreement, including the reporting requirements. ‘The second tranche will be released after the Grantee has submitted the completed and approved financial report with supporting documents amounting to a least 60% of expenditure ofthe frst tranche. Reduction and Recovery of Grant 1. Without prejudice to SPS's other rights and remedies, SPS may withhold or suspend Payment of any grant funding due tothe Grantee filing to comply with any of the terms ofthis Agreement. 2. ‘The Grantee may not retain any portion of the Grant that has not been used by the end ‘of the Financial Year in the Funding Period, 3. Where al oa proportion ofthe Grant Funding remains unspent atthe end ofthe Financial Year, including as aresul of termination ofthis Agreement, allo @ proportion ofthe Unspent Monies, as calculated by SPS, must be repaid to SPS within thirty (20) working days from the date on which the request for payment is sent and must not be carried forward for use in the following Financial Year. Project Reporting and Financial Management 1, ‘The monthly monitoring report will: ‘a describe the Project activities completed and the results achieved; b. contain an assessment of progress made against the proposal and refer to the indicators of suocess inthe proposal. 2. The financial reports will tate: ‘how much fanding was spent; and '. the purpose ofthe expenditure of the funding in (a) above; ‘When the Project has been completed the Grant will prepare and send a final report (a “Project Completion Report”) 1o SPS within the period that SPS requests. This report will ‘contain a detailed breakdown of all expenditure for the grant funding period. Monitoring and Evaluation ‘SPS will monitor the progress ofthe Project throughout the Funding Period. SPS could schedule online meetings on top of the online monthly check-ins (via Google form) with the Grantee if requested by either party. In turn, the grantee will: 1. make a representative or representatives ofthe project available to meet with and answer questions about the project implementation; and 2. review, with SPS, ift is necessary to refocus project outputs. Authorized Representative 'SPS's Authorized Representatives are Anna R. Oposa, Executive Director and Harvey S. Perello, Project Manager. They have the responsibility to monitor the Grantee's performance, ‘The Grantee's Authorized Representative is Mr. Jessie Talic Mamites. Ifthe Grantee's ‘Authorized Representative changes at any time during this grant contract agreement, the Grantee must immediately notify SPS. Authorized Fund Transfer Channel ‘The Grantee will receive the project grants in the following bank account or fund transfer ‘channel as indicated on the Fund Transfer Request Google Form the the Grantee filled out: ‘Money Transfer Method: Palawan Pera Padala Recipient: Jessie T. Mamites Recipient's Mobile Number: 09153035489 Location: Poblacion, Sibutad, Zamboanga del Norte ‘Thisis cemtfied true and correct by the Grantee: Jessie T. Mamites Ifthe Grantee's Authorized Bank Account or Fund Transfer Channel changes at any time during this grant contract agreement, the Grantee must immediately notify SPS. Liabitity ‘The Grantee must indemnify, save, and hold SPS, its agents, and employees harmless from any claims or causes of action, including attorney's fees incurred by SPS, arising from the performance of this grant contract by the Grantee or the Grantee's agents or employees. This clause will not be construed to bar any legal remedies the Grantee may have for SPS's failure to fulfil its obligations under this grant contract ‘Termination for Cause ‘SPS may immediately terminate this grant contract if SPS finds that there has been failure ‘to comply with the provisions of this grant contract, that reasonable progress has not been ‘made or that the purposes for which the fimds were granted have not been or will not be fulfilled. ‘SPS may take action to protect the interests of SPS, including the refusal to disburse ‘additional funds and requiring the return of all or part of the funds already disbursed. ‘Sibutad National High School Anna R.
