Definition
A graded exercise test (GXT) is a
screening tool to track an individual's
fitness level. The test evaluates the
participant's exercise capacity by
measuring cardiovascular response to
physical activity.
USES:
for patient with high probability of disease
to acess myocardial ischemia
to acess threatning ventricular arrythmias
to detect abnormal b.p
CHD patient (25-40%) have normal ECG, so graded ex
tests are performed
to make rehab program
monitor rehab techniques
Contraindications
recent MI
UNSTABLE Angina pectoris
uncontrolled ventricular arrhythmia
3rd degree AV BLOCK
congestive heart failure
Aortic stenosis
Acute infection
Acute emotional distress
Procedure
INCREMENTAL: work rate changes 2-3 min
exercise end when person reaches
predetermnied point or when any symptom
appears
Maximum: 85% age determined heart rate
Heart Rate
HOW TO
MEASURE???? Steady state
By palpation of radial
or crotid artery, using
surface electrode
DONOT APPLY TOO Post exercise
MUCH PRESSURE
measure for 15-30 SEC
Blood Pressure
measured by sphgmomanometer
sthethoscope is placed below cuff and brachial
artery

FIRST SOUND SYSTOLIC

FORTH SOUND DIASTOLIC

cant be measured
during running
subject should not be
holding onto the
handlebar of the
cycle and treadmill
Electrocardiogram
during exercise ECG can be measured by
single bipolar lead
Evaluated for arrythmias(atria, AV node,
ventricles),duction disturbunce(depol is
slow) and Myocardial Ischemia
Perceived
Exertion
Perceived exertion is how hard
you feel like your body is
working.
based on the physical
sensations a person experiences
during physical activity,
including increased heart rate,
increased respiration or
breathing rate, increased
sweating, and muscle fatigue.
ORIGNAL
RATING SCALE
Termination
criteria
Angina like symptoms
drop in systolic b.p
increase in systolic more thn 250 mm of Hg
and diastolic more than 115 mm of Hg
poor perfusion
severe fatigue
change in heart rhythum
