SWORN DECLARATION
We, Spouses MANOLO PAR MARIANO and ANA MARY VENANCIO
MARIANO, both of legal age, Filipino Citizens, and currently residing at Purok
Masagana, Barangay Ibabang Iyam, Lucena City, after having been duly sworn
to in accordance with law, do hereby depose and state, That:

1. We are the legitimate parents of EARL JEDRICK V. MARIANO, who is

applying for a Scholarship Program at Southern Luzon State University
(SLSU);

2. As a jeepney driver with daily income of Five Hundred Pesos (Php500.00)

and a housewife with no source of income, respectively, we are low-
income earners;

3. Our monthly income of TWELVE THOUSAND PESOS (Php12,000.00) is

so meager and only enough to cover our daily needs;

4. We executed this sworn declaration to attest to the veracity of the above

statements and in support to our son’s application for scholarship program
at Southern Luzon State University (SLSU).
