On December 26th, 2022 time 10:00 AM while BVI-
HSE Performed daily site inspection, found unsafe
action PIBK workers who were doing sprinkler pipe
painting work standing on top-rail scaffolding,
potentially causing workers to fall down and get serious
injuries

On December 26th, 2022 time 10:03 AM while BVI-
HSE Performed daily site inspection, found unsafe
action PIBK workers who were doing wall plastering
work did not wear special PPE (full-body harness)
when working on scaffolding, potentially
causing workers to fall down and get serious

condition the wheels on the mobile scaffolding were with good ones
damaged, potentially causing the scaffolding to collapse

condition the safety railing on the work platform only
uses rebar, potentially unable to withstand the weight of
workers when falling down

On December 26th, 2022 time 14:19 PM while BVI-
HSE Performed daily site inspection, found unsafe the protruding rebar has been cut
condition the protruding rebar was not cut immediately,
potentially causing someone to stumble
Location: Ramp and pump room

On December 26th, 2022 time 16:04 PM while BVI-
HSE Performed daily site inspection, found unsafe
action PIBK workers who were cutting HT (granite)
using angle grinding that was not equipped with wheel
guards and also did not wear PPE (face shield or safety
goggles, earplugs, and safety gloves), potentially
causing workers to be injured when the cutting disc
break
