I, PHILIP SCOTCH ALAGDON OCAMPO, Filipino citizen, of legal age, with

postal address at #3106 Ilang-ilang Street, Hensonville Subdivision, Angeles City, after
being sworn in accordance with law, hereby depose and state that:

1. I am currently employed as Teacher I of Malabanias Integrated School (MIS). I started

working in the said school on May 2012 up to present;

2. Considering that I am applying for Master Teacher I in the said school, I have submitted
and attached herewith pertinent documents as a requirement for the said position;

3. I am executing this Certification to attest to the authenticity and veracity of all said
documents submitted;

4. Therefore, by executing this Omnibus Certification of Authenticity and Veracity of

Documents, I hereby authorize the Department of Education, Division of Angeles City, to
verify the authenticity of the said documents;

5. Finally, I am executing this affidavit to attest to the truth of the foregoing statements and
for all legal intents and purposes it may serve;
