Education Merit Selection Plan which was issued last April 22,2022.

a. Letter of Intent addressed to the Head of Office, or to the highest human

resource officer designated by the Head of the Office
b. CSC Form 212 (Revised 2017) in two copies with the latest 2x2 ID picture
c. Certified photocopy of certificate of Eligibility/Rating/License/ID
d. Certified photocopy of scholastic/academic record, such as but not limited to
Transcript of Records (TOR) and Diploma
e. Photocopy of Service Record or Certificate of Employment, if applicable
f. Photocopy of Certificates of Training, if applicable
g. Photocopy of the latest Performance Rating covering one (1) year performance,
if applicable
h. Omnibus certification of authenticity and veracity of all documents submitted,
signed by the applicant
i. Checklist of Requirements
j. Other documents as be required

2. I am executing this Certification to attest to the authenticity and veracity of all

documents submitted.
3. I understand that any violation will automatically disqualify me from the
selection process.
4. By executing this Omnibus Certification of Authenticity of Documents, I
hereby authorize the Department of education, SDO Santiago City through the
School Head, to verify the authenticity of the abovementioned documents.
