1. I am applying for a teaching position at San Mateo General Comprehensive

No. 3, s. 2016 re Hiring Guidelines for Senior High School (SHS) Teaching Positions
Effective SY 2016-2017.

he/she intends to teach.
B. CSC Form 212 (Revised 2017) in two copies with the latest 2x2 ID Picture
C. Certified photocopy of certificates of relevant specialized trainings
D. Certified Voters ID and/or any proof of recidency
E. NBI Clearance
F. Omnibus certification of authenticity and veracity of all documents
G. Certified photocopy of Diploma on Bachelor's degree
H. Certified copy of Transcript of Records (Undergraduate, Masteral, Doctoral)
I. Certified photocpy of PRC professional identification card
J. Certified photocopy of ratings obtained in LET or PBET
K. Copies of service record, performance rating (SPED applicant), certificate of
employment
L. Copy of Published work, Recognized innovations and accomplishments in
research and development, Prizes, awards, recognitions, letter of
commendations for theses or major academic paper, exemplary performance,
work and/or academic projects. Practitioners/experts may also submit
Government-issued business permit (if self-employed), Certificate or proof of
proficiency issued by duly recognized body including guilds and sports
organizations, Certificated of recognition from duly recognized body
including guilds and sports organizations, Certificated of accreditation from a
duly recognized body including guilds and sports organizations.

N. TESDA Trainers Methodology Certificate (TMC)
O. Certification of Proficiency/Recognition from recognized and respectable
relevant associations/organizations/guild
3. I am executing this Certification to attest to the authenticity and veracity of all
documents submitted.
4. I understand that any violation will automatically disqualify me from the selection
process.
5. By executing this Omnibus Certification of Authenticity of Documents, I hereby
authorize the Department of education, SDO Isabela through the School Head, to
verify the authenticity of the abovementioned documents.
