2.8 Certificates of Outstanding Accomplishments (Awards, Innovations, Research Projects, publications,
Speakership).
2.9 Certificates of specialized training, scholarships grants, chair/co – chair of the technical working
committee
2.10 Psychological/ Neuro- Psychiatric Examination (must be submitted before the completion of the Pink
Folder duly sealed)
2.11 Omnibus certification of authenticity and Veracity of the documents submitted, signed by the
applicant/s.
2.12 Data Privacy Consent Form
2.13 Other pertinent documents to support the application.

3. I am executing this certification to attest to the authenticity and veracity of all documents submitted.

4. By executing this Omnibus Certification of Authenticity and Veracity of Documents, I hereby

authorize the Department of Education, Division of CEBU PROVINCE , to verify the
authenticity of the abovementioned documents.
