This is in connection with the request of the Sangguniang Barangay of Liwayway and Sangguniang Bayan of the
Municipality of Santa Rosa in the Province of Nueva Ecija regarding conversion of Luis Gonzales Elementary
School with School ID No. 159027 into Luis Gonzales Integrated School.

We restpectfully submits the within pertinent requirements provided under DepEd Order No. 40 s. 2014
regarding said request and asking for favorable action of the higher DepEd authorities on this request for the
