    NOTICE TO EXPLAIN

    TO : Mylene Del Monte

    RE : Failure to Submit Minutes of Meeting

    DATE : January 23, 2019

    Last January 7, 2019 was the meeting at Lipa Office, regarding Accomplishment and Collection
    Report. You were assigned to submit and cascade the Minutes of The Meeting to Main Office and
    Site Engineer. However, you have failed to submit the Minutes of the said Meeting.

    With this, you are hereby directed to provide a Notice of Explanation on or before January 25, 2019
    addressed to our Human Resource Department detailing the reasons for this failure.
