The above-named lady was admitted to Hospital Miri from 23.5.2023 until 25.5.2023 for Left
Lacunar Infarct - pure motor, NIHSS 8. She was presented with right hemiparesis for 6 days prior to
hospital admission. CT Brain dated 23.5.23 showed multifocal chronic infarcts (left centrum
semiovale, left corona radiata & left lentiform nucleus). ECG noted sinus rhythm. She has
underlying Hypertension and Ischemic Heart Disease. ECHO dated 2.1.2020: Ejection fraction
46%, noted RWMA. She was an ex-smoker.

2. During the review in Hospital Miri general ward on 24.5.2023, we noted that her cognition
grossly intact, she was oriented to time/place/person and able to obey 3 steps commands. Her
speech noted mild dysarthria. She tolerated oral feeding. Physically she had right hemiplegia, her
motor power of RUL 0/5 and RLL 0/5, LUL 5/5 and LLL 5/5. Sensory intact. Functionally she
required one man moderate assistance in sitting up from lying position and in transfer. She also
required moderate assistance in personal Activities of Daily Living. Modified Barthel Index scored
62/100.

3. She is married and has 3 children (second union). She stays with family in Long Banga. She
is keen for therapies at Klinik Kesihatan Long Lama. She has no equipment, her husband plan to
loan standard wheelchair.

4. Kindly refer physiotherapy for carer education and training for transfer training, gait and
balance training with aid. Also kindly refer occupational therapy for pADL assessment and
retraining. Her appointment at Hospital Marudi Visiting Rehab Clinic was given on 24.7.2023 at
10am. Thank you.
