1. An extension of accreditation period is granted to those whose birth dates fall between

I April1 and December 31, provided that they apply for accreditation 90 days prior to
the deadline for the renewal of accreditation.

2. For those whose birth months are fro11.1. January, February and March, the applicacion
I for renewal should be submitted not later than October 31, N0wember 30 and
December 31, 2000, respectively.

I 3. For those whose birth months are from April to December, the applications for
renewa~ ~f acc~editationshould b~ subrnit;ed on or ~efore Jarr-uary 31, ~ebruary 28,
