Sample Submission Instructions:

1. Pick up blood tubes & a hard copy of the Test Requisition Form (TRF) from the collection site. Confirm each tube is labelled
correctly.
2. Place the TRF & tubes inside a Biohazard bag & seal it well.
3. Wrap the whole thing with plenty of bubble wrap for protection.
4. Place the bubble-wrapped sample and gel pack(s) within a small Styrofoam box. Add bubble wrap or other cushioning material
to fill empty space.
5. Place the small Styrofoam box within a bigger Styrofoam box, with more gel pack(s). Add bubble wrap or cushioning material to
fill empty space.
6. Place the Styrofoam box within a cardboard box.
7. Contact local FedEx/UPS to complete the shipping label & request a pick-up.

We highly recommend including a data logger and wrapping it along with the blood tubes(filled with dummy samples) to monitor the
temperature throughout the shipping test. (Refer to the picture as attached).
We also recommend using Aluminum tape to wrap the small-size Styrofoam box.
