Why we chose this project:

 We chose this subject to raise awareness about discarding

urine bag in a proper way in order to maintain infection
control.

 Also we have noticed how they discard proper way urine bags
collection in Al Mizan hospital, and we think that it’s not the
right healthy way.
Objectives:

1.To Identify the term infection control and reduction.

2.To understand the importance of urine bag collection and disposal.

3.To Minimizing common mistakes on urine bag collection. 4.To

Identify the harmful results that occur as a result of not dealing with
the collection of urine bags.

5.To Work on a radical and suitable solution to the problem of urine

bags.

Introduction

We are students of Al-Quds university in management course in Al-

Mizanhospital , while our training in the hospital , we had seen a
wrong way to emptying urine bag , according to that we decide to start
making a change project of urine bag collection .

Experts said that negligence in the application of means of infection

control within hospitals leads to a prolonged stay, and open the door
for the emergence of new types of bacterial infection , and may raise
death rates, and significantly affect the quality of medical work, in
addition to increasing the cost and occupancy Hospital beds.

The way that nursing followed it in get rid from urine bag is wrong,
because the way that follow is one of three ways in the hospital:
1- First way is emptying the urine bag in the bathroom then throw
it (empty) in the medical waste box.(without perceptional
measurement)
2- Second way is throwing the urine bag in the waste box (not
medical waste )
3- Third way is emptying the urine bag in urinal then throwing it in
(empty in the medical waste)

This previous three ways is need to develop according to CDC(Medical

wastes require careful disposal and containment before collection and
consolidation for treatment. OSHA has dictated initial measures for
discarding regulated medical-waste items. These measures are
designed to protect the workers who generate medical wastes and
who manage the wastes from point of generation to disposal).967

1-The first way is wrong because it not right to emptying the urine bag
in the bathroom because it put the nurse who emptying the bag under
the danger to be affected by urine ( it may cause infection to the nurse
or the urine can splashing on his uniform making it dirty and
infected ).Although , this way may affect the patient because as I
mentioned the urine may splashing but not only on the uniform it may
splash on the walls or boxes in the bathroom ,so if the bathroom
doesn’t clean well the microorganisms maybe affect another patient
and cause nosocomial infection.

2-Second way had been rejected by municipal .

3-Third way also make a danger for contamination .

Another reason why we chose this project to change , according to

some head-nurses request because they see it as a real problem that
need to solve .

To do the project , in the beginning we need to know the definition of

urine bag collection and detect the purpose of doing and choosing this
project .
Definition :

Urine drainage bag collect urine, that bag will attach to a

catheter( tube) that is inside to the bladder, may apply a catheter and
urine drainage bag because there is a urinary incontinence, urinary
retention ( not being able to urinate), surgery that made a catheter
necessary or another health problem effect on the urinate.

Urine bag collection is described as

 Urine Drainage system for short as well as long term Use.

 Super smooth kind resistance tube provided with universal
tapered connector.
 Specialty designed hook or hanger facilities for carrying, handling
and holding the tube in upright position.
 Efficient non return valve prevents the back flow.
 Drainage Outlet System.
 Bag graduated in ml to indicate the quantity of urine collected.
 Transparent sheeting allows visual inspection.

Purpose:
1. To protect the hospital from any violations by the ministry of
health or any official party responsible .
2. Protect patients and medical staff from infection and thus
protect it from liability
3. To maintain the reputation of the hospital in terms of health
services
4. To be ideal in terms of action and application of infection control
standards

Problem statement

The problem of the hospital is a repeated problem in all department its

way to get rid of urine bags for patients. Their dimmer is the throwing
of these bags in the cascade of the trash. This is our view of the wrong
behavior and a non-commented view of the disk of them who are
disposed of and disposal of this market with her side by a border on
the hospital in the hospital we will show the effect on it all on them all
on them and then solution to this speaker in the best way as possible
and comfortable and his dead as a format.

Why Hospitals Need a Strategic Plan for urine collection?

 the strategic planning process allows an organization to strike a

much needed balance between managing the short-term needs
of patients and planning the long-term goals of the organization
to ensure patient safety.

 Safe urine collection use requires careful planning and cannt be

achieved if all of the organization’s resources are spent meeting
the patient’s immediate needs.
 use time management , decrease worst time .
 Decrease stress in nurses during critical cases .
 decrease of risk harmful or death , and decrease risk urine
collection
Strategies to avoid errors involving urine collection
 Empirical – rational strategy
 Based of knowledge (no authority).
accept \ reject
 often effective when little resistance to the Proposed
change is expected and the proposed change is expected
and the change is perceived as reasonable .
-new technology
- easy to use .
- Improves quality of care.
- Saves the time .

: Planning strategy

Inputs : the MW and SW in these hospital rejects empting urine bag in

the bathroom , after the municipality is invoke about don’t make the
. ideal method , the stuff applying the ideal way for 3 Monthes just

; Activities

. the solve is (put medical waste in every room )-1

. we start to getting acceptance from target director-2

we making budget about cost of medical waste (number & material -3
which used in )/ the price for one is 50 shekel we can used PP5 plastic
. * polypropylene * it is more healthy

we choosing the place which the medical waste put on there with -4
. suitable measurements

. " we used labeling or "danger marker – 5

we making good health education for patients &banders about -6

. using medical waste

; Outputs :the Ideal method as GCI is

should be done wearing non sterile gloves and via the drainage tap at *
. the bottom of the bag

when the bag is empty , the tap should be closed securely and wiped *
. with a tissue . if the tap does not have a tap ,than replace it when full

. do not disconnect the bag to empty and than reconnect it *

a separate urine bag collecting receptacle must be used for each pt *
. and each bag should emptied separately as required

for purposes of measuring urinary output , an integral measuring *

. device is necessary

the urine receptacle should be heat disinfected if possible and should *

. be stored dry after each use

after empting the receptacle ,the gloves should be discarded and *

. hands washed and dried thoroughly

contains M.O
put the urine

— Change theory:

— Lewin’s theory

— We choose the lewin’s theory for our project , to improve quality of

critical cases and reduce\ avoid error

— His most influencial theory was his model of the change process in
human systems.
— Kurt Lewin theorized a three-stage model of change that is known
as the unfreezing-change-refreeze model that requires prior
learning to be rejected and replaced.
— Lewin's theory states behavior as "a dynamic balance of forces
working in opposing directions. "

STAGES
Consists of  three distinct and vital stages:

1. “Unfreezing”

 Unfreezing is the process which involves finding a method

of making it possible for people to let go of an old pattern that
was counterproductive in some way.

 Unfreezing is necessary to overcome the strains of individual

resistance and group conformity.

 Unfreezing can be achieved by the use of three methods.

o First, increase the driving forces that direct behavior away

from the existing situation or status quo.

o Second, decrease the restraining forces that negatively

affect the movement from the existing equilibrium

o Third, find a combination of the two methods listed above.

2. “Moving to a new level or Changing” or Movement

 This stage involves a process of change in thoughts, feeling,

behavior, or all three, that is in some way more liberating or
more productive.

3. “Refreezing”

 Refreezing is establishing the change as a new habit, so that it

now becomes the “standard operatixem and motivate it .

4- Set plan .

Move (change)

In this phase ,we work together to apply plan to achieve success

change and clarify the defferences between old and new system, and
reduce the infection and keep the respect the nurse in department

Actually we worked on design form of new plan , to be easier to

use , to meet the need, and to use Properly
and we discussion to do a special new medical waste , and
remove urine collection in bathroom and remove the urine bag
by right standard precautions in special new medical waste and
training for all health care professionals in new policy .

— Development better monitoring systems for urine collection

errors and immediate intervention urine bag .

REFREEZING
When the changes are taking shape and people have accept the new
ways of working, the organization is ready to refreeze. We go to
making sure that the changes are used all the time, and that they are
inserted into work . After implantation the new system we make sure
to do evaluation of new system.

APPLICATION

 It is pertinant that the driving and restraining forces must be

analyzed before implimenting a planned change.

Contingency

In this project we have developed a solution for the problem of urine

collection in the hospital departments but if this solution is not
desirable by the nursing team and by the hospital administration we
have developed several solutions for the collection of urine as an
alternative plan for the main solution to the problem if there is an
objection to the solution We will show these solutions so that this
project does not fail.
The alternative solution is put medical waste in the stuff room on the
open department with yellow color and closed department with red
color, and use biohazard bag with portable trolley should be labeled
with clear words clarified what the goal to used it. and we should have
response person to check it and evaluation stuff works.
Recommendations
1. Check urine culture to determine correctly method to emptied
urine bag.
2. Makes good health education for patients and visitor.
3. Teaching the nursing stuff about important of infection control.
4. Evaluation the application of new method by head nurse and
ND.
Summery

— Based on what has been discussed about the problems

related to this topic we hope all those responsible for
solving this acute problem by taking correct measures
at the level of patients and staff working in the hospital
departments so that we can as much as possible and
significantly reduce the problems related to the
collection of urine bags .... Consider as we explained to
you to empty these bags before disposal to reduce all
levels of infection and thank everyone for listening...
