AFFIDAVIT OF UNDERTAKING

I, JACKIELYN MAE TANAQUIN, of legal age, Filipino, residing in

______________________________________________________________________, having
been duly sworn accordance with law, hereby depose and say:

1. That, pursuant to a lease contract, I voluntarily acknowledge that I owe

Mr. Celedonio Solano the total amount of ONE HUNDRED FIFTY FIVE
THOUSAND and FOUR PESOS (P155,004.00) for leasing his property
located in Novaliches, Metro Manila.

2. I am voluntarily undertaking myself to pay my balance for the rent in

increments starting May 1 2023, I shall begin paying the amount of
FOURTEEN THOUSAND PESOS (P14,000.00) at the beginning of every
month and on each successive months until May 2024 or until full
satisfaction of my balance.

3. I execute this affidavit to attest to the truth of the foregoing facts and to
undertake payment of the said amount to Mr. Celedonio Solano.

4. Failure on my part to return the amount stated within the allocated

monthly period, shall give Mr. Celedonio Solano causes of action against
me including but not limited to the filing of civil and criminal cases.

That I signed this affidavit to attest to the truthfulness of the foregoing

statement.
