After final hearing, upon finding by the Court that the defendant is mentally ill
and that the defendant's being at large poses a real and present threat of substantial harm
to himself and/or to others, it is ORDERED that the defendant be involuntarily
committed to the custody of the Commissioner of the Department of Mental Health and
Mental Retardation of the State of Alabama for treatment and therapy.

It is further ORDERED that the Commissioner of the Department of Mental

Health and Mental Retardation shall submit reports on the defendant's status every
_____________ days to the Court, the District Attorney, the defendant and the
defendant's guardian or attorney. The defendant shall not be released from custody nor
permitted to be at large without supervision and attendance unless specifically authorized
by the Court.
