Stage 2: Email Template

Send the following email to the Witness(es) and Respondent(s) who will be interviewed:
Dear [Name]

A workplace investigation is currently being conducted and you have been selected for an interview as a
[Witness / Respondent]. Unfortunately, I am unable to disclose any details at this time; however, I will be able
to brief you more at the interview.

The interview may take up to one (1) hour.

You may bring an independent person to the interview. If you do, you must advise them that they are there is
a support capacity and not to provide advocacy. They cannot interrupt the interview and they may be
requested to leave if the meeting touches on issues involving information sensitive to Service Stream.

If applicable, let me know at your earliest convenience who your support person will be.

Please note you must maintain confidentiality both during the investigation and after it. If anyone asks you
about the investigation, please refer them to me.

Send the following email to the Complainant:

Dear [Name]

As you know, a workplace investigation is currently being conducted as a result of your formal complaint. The
interview may take up to three (3) hours.

You may bring an independent person to the interview. If you do, you must advise them that they are there is
a support capacity and not to provide advocacy. They cannot interrupt the interview and they may be
requested to leave if the meeting touches on issues involving information sensitive to Service Stream.

If applicable, let me know at your earliest convenience who your support person will be.

Please note you must maintain confidentiality both during the investigation and after it. If anyone asks you
about the investigation, please refer them to me.
