BENCH WARRANT OF ARREST

TO ANY OFFICER OF THE LAW:

You are hereby commanded to arrest FRANCILLI RAFAEL DELA CRUZ , who is/are said to
be found in/at NO. 83 STANDFORD, E. RODRIGUEZ, QUEZON CITY,CUBAO, QUEZON CITY,
NATIONAL CAPITAL REGION and who stand/s charged before me FOR FAILURE TO APPEAR
BEFORE THIS COURT ON THE SCHEDULED HEARING and to bring him/her/them forthwith
before me to be dealt with according to law.

The bail for the release of the accused in this case is fixed at Php 3,000.00 for VIOLATION
OF ARTICLE 319 OF THE RIVISED PENAL CODE( ) , which may be furnished by the accused or
a bondsman in the form of corporate surety (Section 10, Rule 114), property bond (Sections 11-13, Rule
114), cash deposit (Section 14, Rule 114), or recognizance (Section 15, Rule 114), to guarantee
his/her/their appearance before this court as required under the conditions specified in Section 2 of Rule
114 of the Rules of Court.

You are required to use at least one body-worn camera and one alternative recording device, or
a minimum of two devices, or such number as may be necessary to capture and record the relevant
incidents in the execution of this warrant pursuant to the Rules on the Use of Body-Worn Cameras in
the Execution of Warrants (A.M. No. 21-06-08-SC) issued by the Supreme Court and which became
effective on 1 August 2021.
