In consideration of and for the permission and authority of the CHILD to participate in the New
York City Police Department's NYPD Police Commissioner’s Youth Soccer League (“Event"). I
hereby release and forever discharge, and shall hold harmless and indemnify the New York City
Police Department and the City of New York and its agents, servants, employees, volunteers,
successors and assigns (collectively referred to hereinafter as the “CITY") from all actions,
causes of action, suits, debts, sums of money, accounts, damages, judgments, claims and
demands whatsoever which I or the CHILD, or our heirs. executors, administrators, successors,
employees and assigns may have now or in the future against the CITY arising out of the
CHILD's participation in the Event, including, but not limited to, the aforementioned activities
and any acts related thereto. The CHILD will not knowingly be exposed to danger. Should an
accident occur, I realize that I am responsible for the CHILD's medical care.

and who is a participant in the NYPD Police Commissioner’s Youth Soccer League, sponsored by the New York City
Police Department(“NYPD”). I hereby authorize the NYPD, the City of New York and its agents, servants,
employees, volunteers, successors, and assigns (collectively known as the "Sponsor'') to make visual and audio
recordings of the child's participation in the Event for any and all legitimate purposes. These recordings may be
used in Sponsor brochures, on posters, in films, in electronic such as placement on the Internet/web. as well as
other media and at Event related events.

I agree that Sponsor may videotape and photograph the child and record his/her voice, conversation, and
sounds, during and in connection with his/her participation in the Event. I understand that the Sponsor will be the
exclusive owners for all time of the videotapes, photographs and audio recordings, as well as Event materials that
they are used for. In addition to the child's likeness and voice, I consent to his/her name and biographical material
that the child or I provide being used in the Event materials.

With my consent, the Sponsor has the unrestricted right to copyright and to use all or any portion of the
Event materials that the child appears in and/or is heard in. I understand and agree that there is no limitation on
the number of limes that the Event materials containing his/her image and/or voice may be used by the Sponsor.

I further agree that the Sponsor has the unrestricted right to license others to use the child’s name, voice,
likeness, and any biographical material in any and all media and in the promotion, advertising, sale and publicizing
of the Event by the Sponsor.

I hereby waive any right of inspection or approval of the child’s appearance or the uses to which his/her
appearance may be put. I acknowledge that Sponsor will rely on this waiver and I hereby agree not to assert any
claim against the Sponsor which may result from reliance on this waiver.

I expressly acknowledge that the child has not been promised, nor does he/she expect, either now or in
the future, any salary, bonuses, commissions, royalties, preferences or benefits of any kind as a result of his
participation in the production of these Event materials from the Event, the Sponsor, or any person or entity
associated with the Event.

I waive any and all potential rights, claims. and causes of action for salary. bonus, commissions, royalties,
preferences, or benefits of any kind based on the child's participation in the production of these Event, and
training materials, that the child may have now or in the future against the Sponsor, and all other entities and
persons who are or may be associated with their production, distribution and exhibition.

I have read this release and understand its contents. I have signed it voluntarily.
