1. Highlight Production

Pulp Machine production on July 2021 was 41,909 Adt, which is unfavorable by 1,576 Adt from
the Monthly Performance 40,333 Adt, unfavorable by 734 Adt from the monthly high
performance 42,643 Adt also unfavorable by 4,740 Adt from the Monthly budget 37,169 Adt.
Acacia MPB (Monthly Production Budget) = 7,980 Adt, Actual = 4,937 Adt.
Pellita MPB (Monthly Production Budget) = 29,189 Adt, Actual = 36,971 Adt.

The topic operations during July 2021 are:
a. Pulp Machine circulation and sheet break:
Total circulation on Jul-21 was 2H50M due to:
Folding machine line #1 problem 32M
Slitter no 5 jammed sheet jump to pulper 47M
Slitter no 8 loose 28M
Replace relay #1 & shaft conveyor #2 (7650) 1H3M

b. Jun 2nd, 2021 at 09:30 Stopped Lime kiln due to spark coming out at kiln drive. Found that
the rubber coupling was damage and replaced it then did re-alignment. Started Kiln
heating up at 13:35 and at 15:40 Started feeding (DT 6H10M). On Jun 3rd, 2021 check
kiln vibration 2.69 mm/s while speed 1.75 rpm (still acceptance to operate).

c. Jun 3rd, 2021 at 08:05 Stopped Pulp Machine based on Opportunity schedule. Main job:
Replaced Pick up felt, bed and fly knife, vacuum pump #1, and bearing of circulation fan
#31 and #102 but fail so only re-alignment for both fans. Started machine on 23:15 and
Sheet production at 02:13 (DT 18H8M). Other’s plant stopped as follow:
- ClO2 production stopped at 08:00~21:00 based on schedule, Opportunity job: Acid
cleaning Chlorate cell, replaced chlorate cell #1 and #2, installed “Y” strainer on
Chilled water line, replaced steam bundle gasket of weak ClO3 Evaporator and
replaced cell cover gasket of Chlorate cell #26 (DT 13H).

- Bleaching plant stopped at 08:05~19:20 based on schedule to repaired D0 mixer
foundation, unplugged distributor D0 and EO pulp press and replaced nose MC pump
G-0150 (DT 11H15M).
- Cooking plant stopped blowing at 08:50~11:35 took opportunity to repaired WL
line/LP-steam line to LP feeder and repaired C-1000 rubber skirt (DT 2H45M).
- Washing plant stopped at 09:45~13:25 took opportunity stopped to unplug sampler
post O2 kappa (DT 3H35M).

d. Jun 6th, 2021 at 15:10~21:35 Bleaching plant stopped due to Gland packing of D0 tower
was leaking and boxed up to plug the leaking by plate (DT 6H25M). Last replacement of
Gland packing was Nov-20 ASD and after inspected on Mar-21 MSD indicate shaft sleeve
has little worn out and planning to replaced next Nov-21 ASD.
Meanwhile Washing plant at 18:37~22:28 stopped due to Brown stock HDT level was high
(DT 3H51M).

e. Jun 18th, 2021, all plants stopped due to high inventory after Pulp machine sheet
break and unsuccessful 12 times feeding (sheet moving to front side and break at inside
Dryer).
Countermeasure has done:
- Checked turning roll speed (OK).
- Cone circulation fan #67 was touching with blade and take out.
- Sprocket chain turning roll was loose and re-tighten.
- Replaced switch panel turning roll.
- Tape threading top pulley was fall down and fixed back.
- Tried to reduce the tension due to consider pulp quality after long time circulation (see
the start stop time at sheet break report above).

Others plant also stopped and started as follows:


- ClO2 plant stopped at 09:14~14:10, stopped again at 17:00~02:10 (total DT 14H6M).
Opportunity jobs: Cleaning plate strong ClO3 Chiller Heat Exchanger and flush weak
Cl2 recycle header ClO2 Generator.
- Digester stopped at 11:35~02:12 (DT 14H37M).
- Washing Plant stopped at 15:30~01:30 (DT 10H).
- Bleaching plant stopped at 15:30~00:10 (DT 8H40M). Opportunity job: Welding HP
sealing water line and replaced vacuum pump D1/D2 press standpipe.

f. Jun 21st, 2021 at 12:55~16:39 Washing plant stopped due to Dilution pump 421-G-2530
coupling was broken and caused high consistency at Pre O2 press dilution conveyor. Then
contactor and thermal overload for 421-G-1550 Pre O2 Shredder conveyor was burnt (DT
3H44M). Countermeasure has done: Replaced contactor and thermal overload of motor
Pre O2 press dilution conveyor, replaced coupling of dilution pump 421-G-2530 and
Flushing the line to TRPE.

g. Jun 23rd, 2021, all plants stopped due to high inventory after Pulp machine sheet
break (see the start stop time at sheet break report above).
- Digester stopped at 17:05~04:40 (DT 11H35M).

- Washing Plant stopped at 15:32~03:00 (DT 11H28M).
- Bleaching plant stopped at 11:40~01:00 (DT 13H20M).

h. Jun 25th, 2021 until Jun 26th, 2021, Production was not achieved due to many times Pulp
machine circulation by Bale marker line #1 problem (data not change). After inspected
found that communication to forte was failure. Replaced cable on Jul 26th, 2021 and
waiting next opportunity on Jul 19th, 2021 to calibrate.

i. Update of the month:


Jul 9th, 2021 at 03:12 Power boiler tripped due to tube pipe of furnace wall was
leaking.
Counter measure has done:
- Tubes replacement Jobs: Left sidewall 13 tubes has replaced (No. #76~#88), Right
sidewall one tube rebuilt (No. #80)
- Refractory Jobs: Left side 1.8 m x 1.6 m, Right side 0.6 m x 0.6 m
- On Jul 10th, 2021 at 09:00 all jobs has finished and continue to X ray.
- Started to hydro test at 22:53 but delayed due to strainer of Boiler feed water pump
#1 & #2 need to cleaning.
- Continued hydro test on Jul 11th, 2021 at 02:50 and finished at 06:00.
- Power boiler started heating at 06:45 and synchronized to T/G at 11:00 (DT
55H48M).
Countermeasure to be done:
- Improve tube inspection during Shutdown.
- Improve refractory installation especially at sand feeding side (invite vendor).
- Follow refractory drying curve, according to maintenance manual book during
repairing/heating up.
Meanwhile Pulp machine also load shedding at 03:14, started at 13:18 (DT 10H4M).

Jul 10th, 2021 at 07:34 Pulp machine sheet break due to 3rd top press felt was cut off
and the cut off felt that bring by broke conveyor to the wet end pulper.
Countermeasure has done: Replaced 3rd top & bottom press felt and empty out the
Wet end pulper to removed cut of felt. Job have finished at 17:00. Pulp machine
started at 20:08 and sheet production at 23:23 after three times feeding fail (DT
15H49M).

Meanwhile others plant also stopped due to high inventory as follows:


- Bleaching plant stopped at 10:20~00:10 (DT 13H50M).
- Washing plant stopped at 15:32~01:27 (DT 9H55M).
- ClO2 plant stopped at 21:50~00:54 (DT 3H4M).

Jul 11th, 2021 at 13:29 Twin Roll Press Evolution (TRPE) tripped due to high pressure
torque and speed going to zero then by passed. At 02:35 on Jul 12th, 2021 started
TRPE with water but after check found that bearing DS movable roll was broken.
Stopped TRPE at 13:36 to replace the bearing and finished on Jul 13th, 2021 at 03:00.
Started water run at 06:17 and on line with pulp at 08:50. However, unfortunately
found leak at ND bearing fix roll, MMD will pay attention and keep follow up.

Jul 13th, 2021 at 08:30 Stopped Electro Static Precipitator (ESP) #3 of Recovery Boiler
(RB) due to Yokogawa replaced the terminal board. Started again ESP #3 at 17:30
(DT 9H).

Jul 13th, 2021 at 04:19~05:39 CA plant tripped due to control valve PIC-2780 Chlorine
to hypo air regulator valve was leaking.

* High Solar consumption at CWD due to increasing rental Heavy equipment for BC #3 and Hauling bark
from outside.
* High WL consumption for AM due to there was mix with Pellita.
* Kappa Number standard deviation was above target due to big variation of Chip bulk density and difficult
to control Chip bin level.
* High CA DC power due to some of CA cell has over lifetime and replacement will continue the next ASD.
* High solar consumption for back up while plant start stop by Pulp machine problem and BL shortage.

4. SAFETY ISSUE

Working Accident Occurred:


 Zero Accident and Fire for June 2021

Highlights, Issues and Action Plans:

 Checked fire hydrant and fire extinguisher at Mill site and follow-up if found some
abnormality.
 Measured working permit and gases contain for vessel entry permit.
 Conduct safety representative, training, sharing by Siloam hospital regarding vaccine.
 Gave safety induction to PT NPE (21), PT SAA (6), PT Herissa (8), PT Indo Scale (7), PT.
Radian (3), PT. IBM (2), PT Media (3), PT. Citra Trikarya Mandiri (4), PT. SMCC (13) dan
PT Pratama Graha (1) and PT. Yogma (11).
 Do safety inspection at Gate XII
 BOD Safety Patrol at area Admind Building (Truba), Fabrication Shop, MMD Shop and EID
Shop, Chip Screen and Chip pile
 Checked Emergency Evacuation System at MCC Room Cooking and Bleaching, result
good condition.
 Checked fire alarm system at Cooking and Bleaching, result good condition
 Checked safety shower at Cooking and Bleaching, result good condition

 Checked Hoist Crane at Top Separator, Digester Elevator, Loading Dock #1, #2, #3 and
Forklift at Pulp Warehouse, result good condition.
 Accompany Manpower Department and third party for renewal operational permit for Lift
and Transport Equipment, Pressure Vessel, Tank, Boiler, Fire Equipment Installation,
Electrical Installation and Lighting Rod Installation.
 Support and help CWD team due to van belt burn at bark Crusher #4 caused by van belt
slip from pulley.

B. DEPARTMENT ISSUE

1. Chip & Wood Department (CWD)

a. CWD issue
- Continue repairing Log yard #10, #20, #30, #40, and #50. Follow up by POD and EGD.
- Jun 2nd, 2021 at 20:27 Screw reclaimer #4 stopped due to casing gearbox was
damage. Countermeasure has done: Replaced gearbox, coupling, chain transmission
and housing bearing, then inspected bolt screw to sprocket. Started Screw reclaimer
#4 on Jun 9th, 2021 at 21:30.
- Jun 8th,2021 at 20:30 Screw reclaimer #7 stopped due to bearing of screw was
damage. Started screw reclaimer #7 on Jun 8th, 2021 at 18:00.
- Truck scale #4 was down due the bridge construction fault (Load cell foundation was
broken, and H-beam was bending). Replacing the bridge construction started on Jun
4th, 2021 this month and continue until now.
- Jun 30th, 2021 Truck scale #5 started after replacement of the load cell. Truck
scale #5 was down since May 6th, 2021 due to lighting while heavy rain and on May
10th, 2021 started to replaced load cell (total 10 pcs).
- CWD production below than target due to production slowly based on Log inventory.

2. Production Department (PDD)

a. PDD issue

Cooking & Bleaching (CB)


- Found many metal gram from D1/D2, EO & 1 st post pulp press instead the oil leaking
from bearing seal since Apr-21 and added oil around 40 liters/week (1st press), 60
liters/week (EO press), and 60 liters/week (D2 press) then decided to replaced oil back
to Bechem on Jun 3rd, 2021. Currently still adding oil about 15 liters/4days (D2 press),
5 liters/4 days (EO Press), and 1st post 5 liters per week.
Bearing spare status: two pcs available, two pcs ETA, May-21 open PR to purchase
four pcs plan to replace Nov-21.
- Update Spare of Mechanical seal for MC-pump TRPE stand pipe. For new spare
one set still PO (PO 24115, ETA Mill site 10-Aug-21 John crane). New spare of contact
face and casing stationer from Rolon that was arrived on May-21 had already used.
Now available spare is contact face (graphite), and casing stationary.

Pulp Machine (PM)


- After check and repair until Jul 10th, 2021, Circulation fans status were 64 fans in
good condition, 26 fans were alarm #1, 11 fans were alarm #2 (No. #7, #17, #24, #31,
#43, #44, #49, #51, #73, #91 & #102) and 3 fans were not running (No. #30, #46 &
#100).

Effluent Treatment plant (ETP)


- The Aerator #7, #11 and #12 were still down due to screw impeller was unbalance,
cone was broken, and motor got low megger. Need to speed up spare by MMD and
POD PO 24576, ETA Mill site 10-Sep-21).
- On May 2nd, 2021 empty out the holding pond area was started due to planning to
clearing and dredging the pond for normalize the function as previously (waiting JS
approval).

Power Boiler (PB)
-Raw water pump #1 not working properly.
Countermeasure: Replaced module RW pump#1 with new one but on Jun 24 th, 2021
pumps tripped 4 times and on Jun 28th, 2021, replaced 1GBT module for R, S, T of the
pump

Recausticizing Plant (RL)


- On Jun 11th at Lime kiln ESP at field #2 alarm Ans- and suspect a short circuit. On Jun
12th, 2021 at 14:40~18:10 stopped Lime mud feeding to replace the relay for
communication to DCS, but after check transformer still low megger and decided to
check inside ESP and found 1 electrode out of contact with the collecting plate causing
a short circuit.

Recovery Boiler (RB)


- Jun 9th, 2021 PT. TEL has meeting with Andrizt regarding ESP #3 performance with
topic as follows:
Andrizt gave explanation about ID fan #3 backpressure.
Decided to conduct performance test on Jul 7th until 9th, 2021.
Yokogawa also replaced terminal board on Jul 13th, 2021.
- On Jun 28th, 2021 HBL tank #2 was a blockage and the agitator tripped. Isolated and
empty out the tank and cleaning inside. After the agitator rotate and cleaned the level
sensor, the HBL tank #2 was close and still continue soaking by Hot water normally.

4. Chemical Plant Department (CPD)

a. CPD Issue
- High weak acid temperature & Low HCl #2 product concentration due to suspect
top carbon block was plug up by broken glass while inner and outer tube broken.
Countermeasure: Temporary weak acid temperature kept less than 65 oC and increase
HCl #1 product concentration to 33~33.5%. Countermeasure to be done: Overhaul the
unit on the next ASD Nov-22.
- ClO2 absorber #2 tray segment #1 and #2 were damage (include bottom tray support).
Countermeasure to be done: Replace absorber tray bottom support (PO 24124, ETA
Oct-21 for 3 segments) and Replace weak Cl2 blower #1 (PO /TEL/ 025848, ETA Oct-
21).
- Strong ClO3 Chiller performance due to Chlorate chiller HE dirty by chilled water pipe
got corrosion. Countermeasure to be done: Replace the pipe with SS material on the
next ASD Nov-21.
- High vibration stage #1 Air compressor O 2 plant due to Rotor impeller was un-
balance that will affect to trip and risk to continue to run the unit. Counter measure:
Purchase new set rotor impeller stage #1 (PO 22316, ETA End of Jun-21).
- Chiller Unit #1 was low performance as lifetime issue and need to re-new absorption
chiller (PO 23381, ETA End of Aug-21).
- ClO2 Production were stopped 3 times on June 2021 due to T/R DC permissive
tripped by interlock to problem at HCl synthesis unit #1. Many countermeasures have
done such as follows:
Jun 2nd, 2021 Replaced DI module channel, Reset PLC panel and cleaning
touchscreen display.
Jun 6th, 2021 Check sight tube of flame detector and cleaning sight glass of flame
detector.

Jun 7th, 2021 Replaced flame detector, replaced solenoid H2 safety valve and
replaced H2 relay.

- Project land fill #6 has started on October 2020 (progress 73.54% per Jul 8th,2021),
delayed 26.46% compare to plan 100% due to rainy season, land slide, waiting material
leached layer and remove black and grey soil.
- On Apr 1st, 2021 Embankment of Land fill #5 was land slide at position beside of Project
land fill #6. Repairing work by SMCC and EGD on progress 18.5% behind of
schedule as 39.6%.
