Instructions for 3949-A and 211

If you have a corrupt judge or clerk who are aware that they are harming you, but don’t seem to care, then you
can use IRS Forms 3949-A and 211 to initiate an IRS investigation on them and the court they work for. Simply
by reading the attachment for the 3949-A you can grasp what is taking place in the courts. Use the following
attachments for these forms:
a. “Mandatory Attached Statement to the Information Referral FORM 3949-A”
b. “Mandatory Attached Statement to the Information Referral FORM 211”
File the 3949-A with attachments and 211 with attachments at your local IRS office and get it time stamped.
Take two copies of it, one for them to keep that is time stamped and one that belongs to you for them to time
stamp, to keep for your records. (Be sure to scan this and save it to your computer, and have a backup saved on
a hard drive or thumb drive. Even emailing it to yourself to search for later on is a good way to store documents.
This has saved me a lot of time in the past.)

It is recommended to send a Cease and Desist Letter in response to trademark infringement, to include as
evidence that you attempted to get this resolved with the company first. Your trademark serial number is used
on both IRS forms as reference. Trademarks are highly protected in commerce. Scan a copy of the signed letter
showing all of the tracking info and also scan the green return receipt card that comes back to you in the mail
within a few weeks to a month later. You can go to USPS.com and get proof of delivery there to use as evidence
due to the return receipt card taking so long to be returned if you are trying to file the IRS forms sooner than
