PART ONE.
PRELIMINARY PROVISIONS

Section 1. INTRODUCTION

This document shall be known as the USTP Security and Safety Policies and
Procedures, otherwise known as the “USTP Security Policies”, adopted by the
Board or Regents pursuant to BOR Resolution No. __, series of 2022.

Section 2. USTP VISION

A nationally-recognized Science & Technology University providing the vital link


between education and the economy.

Section 3. USTP MISSION

● Bring the world of work (industry) into the actual higher education and training
students;
● Offer entrepreneurs the opportunity to maximize their business potentials
through a gamut of services from product conceptualization to
commercialization;
● Contribute significantly to the National Development Goals of food security
and energy sufficiency through technology solutions.

Section 4. USTP QUALITY POLICY

We are committed to provide our primary customers with excellent and


continually improved quality human resources and services that comply with
statutory and regulatory requirements in seeking to attain sustainable
development and inclusive economic growth as a nationally-recognized
University of Science & Technology.

Section 5. STATEMENT OF USTP SECURITY AND SAFETY POLICY

The Campus Security and Safety Offices (CSSOs), as the main implementer of
the USTP Security Policies, shall:

(1) Provide adequate control against safety and security risks that may arise from
day-to-day activities in the Campus.

(2) Consult with students, employees or personnel on matters affecting their


safety and security.

(3) Provide security and maintain safety within the working environment and
inside the Campus.

(4) Provide information, instruction, and supervision for security and safety
personnel.

(5) Ensure that all security and safety personnel have adequate training so that
they may be competent to do their tasks.

(6) Prevent accidents and cases of work-related illnesses and injuries.

(7) Review and revise standing policies as may be necessary at regular intervals.

Section 1. PERSONAL IDENTIFICATION AND ACCESS CONTROL

1. Only after proper personal identification and showing of legitimate reason shall
security personnel admit any individual to the Campus or any of the buildings
therein.

2. Concerned security personnel shall implement access control measures


among all persons coming inside the Campus.

3. Continuous orientation among all security personnel shall be done by security


chiefs, managers or supervisors to discuss who will have access to the Campus
and buildings, and how the access shall be granted.

4. Those who have not shown any proof of personal identification and legitimate
reason may be denied admission; provided, that incoming vehicles shall not
block the entrance gate for purposes of implementing personal identification and
access control.

5. If necessary, call nearest law enforcement/emergency office if something is


found to be suspicious.

(Please provide here the contact numbers of nearest law


enforcement/emergency offices which shall be posted in conspicuous places in
the Campus).

A. STUDENTS ENTERING THE UNIVERSITY

A.1 Any student entering the Campus on foot shall be required to pass
through the pedestrian gate which may contain a built-in auto counting bio-
register machine and temperature reading gadget. Campuses that have no
counting/monitoring machine, may do random manual checking of
identification.

A.2 Students entering the Campus in a motor vehicle shall be required to
halt at the designated choke/check point wherein identification may be checked
by the assigned security personnel.

A.3 New students or those who are not yet issued a valid student ID may be
required to register at the designated students’ identification logbook.
Enrolment credentials may be checked in lieu of the identification card.

A.4 Security personnel shall enforce any relevant policies that may be
provided in the USTP Student Handbook with respect to entry to and residency
of any student in the Campus. In this regard, security personnel shall undergo
continuous seminar or lecture on this aspect.

B. VISITORS ENTERING THE UNIVERSITY

B.1 Any visitor entering the Campus on foot will be required to pass through
the pedestrian gate which may contain a built-in auto counting bio-register
machine and temperature reading gadget. Campuses that have no
counting/monitoring machine, may do random manual checking of
identification.

B.2 Visitors entering the Campus in a motor vehicle shall be required to halt
at the designated choke/check point wherein identification shall be checked by
the assigned security personnel.

B.3 Visitors with no valid ID may be required to register at the designated


visitors’ identification logbook.

B.4 If a visitor is bringing a bag or any similar thing, he/she may be required
to open the bag. If so, the assigned security personnel shall require the visitor
to open the bag by himself/herself, after which the handling security personnel
may examine the contents therein by using a stick.

B.5 In attending a suspicious person, a body frisking procedure may apply.


Frisking for suspicious women or minors shall be done by lady security
personnel. All duty security personnel are mandated to know this security
deterrence procedure professionally.

B.6 As a general rule, visitors are not allowed to proceed to any of the
buildings in the Campus without a visitor’s pass to be issued by the duty security
personnel, upon surrender and exchange of a valid identification card.

B.7 Visitors are prohibited to meet any teacher or employee or personnel
when the latter is active in a class or work. It is highly recommended that visitors
shall set prior appointment with teachers or employees or personnel.

B.8 Errant drivers who are admitted in the Campus, and those who do not
adhere to the prescribed safety and security policies, may be apprehended and
be turned over to any local law enforcement authorities.

C. EMPLOYEES OR PERSONNEL ENTERING THE UNIVERSITY

C.1 Any employee or personnel entering the Campus on foot shall be


required to pass through the pedestrian gate which may contain a built-in auto
counting bio-register machine and temperature reading gadget. Campuses that
have no counting/monitoring machine, may do random manual checking of
identification.

C. 2 Employees or personnel entering or going out of the Campus in a motor


vehicle shall be required to halt at the designated choke/check point wherein
identification may be checked by the assigned security personnel, except in
extreme necessities such as when in an ambulance or any emergency flashing
vehicle carrying a patient to proceed to a hospital.

C. 3 New employees or personnel or those who are not yet issued a valid ID
may be required to register at the designated employee or personnel
identification logbook. Enrolment credentials may be checked in lieu of the
identification card.

C. 4 Security personnel shall enforce any relevant policies that may be


adopted by USTP with respect to entry to and conduct of all employees and
personnel in the Campus. In this regard, security personnel shall undergo
continuous seminar or lecture on this aspect.

C.5 Exception from the policies on personal identification and access control
for any employee or personnel may be decided by the CSSO on a case-to-case
basis.

D. CONTRACTORS ENTERING THE UNIVERSITY

D.1 Any person having a contract with USTP for any work such as, but not
limited to, construction or repair of a building, painting or re-painting, electrical
or plumbing installation, clogging or de-clogging of drainage system, among
others, shall present to the CSSO an authenticated copy of contract duly signed
by the parties and list of personnel and workers duly signed by the contractor.

D.2 Entry of major construction equipment shall be subject to a gate pass
duly issued by the Infrastructure Planning and Facilities Development Office
(IPFDO) and CSSO.

D.3 Commercial or cargo vehicles that would enter the Campus to deliver
any materials, equipment or goods shall present at the entrance gate duly
signed delivery receipt and/ or purchase order issued by USTP. Before entry
the materials, equipment or goods shall be subjected to systematic inspection.

D.4. Any construction materials that may be withdrawn from the construction
site or out from the Campus shall be subject to a systematic inspection and pull-
out form duly approved by the IPFDO.

D.5 Once inside the construction site, construction personnel and workers
shall remain at their designated work station or working quarter. Access to
students and employees or personnel, with some exceptions as may be
determined by the director of IPFDO, is strictly prohibited.

D.6 On-site Safety Officers of construction contractors shall abide by the


contractual stipulations of its Construction Safety and Health Program (CSHP),
otherwise, the Campus Safety Officer may intervene in the safety operations of
the contractor in the Campus.

E. TERRORISTIC ACTIVITIES

E.1 On vans or motorcycle riders delivering food or any item: these couriers
should be subjected to utmost observation. Guide questions for security guards
assigned in access points: (1) Am I sure that the things inside the delivery box
are really food or commercial items? (2) Am I sure the item inside the delivery
box is not a bomb? These guide questions are in anticipation to any possible
economic sabotage, either or simply making pre-terroristic movement.

E.2 The guard on duty shall ask as to whom the item or food is to be
delivered, and shall call the addressee in order to confirm the delivery. The
addressee/consignee may be required to go to the entrance gate to receive the
ordered items or let somebody do the same on his/her behalf, or allow the
carrier to enter the Campus after being cleared, and see to it that another
rechecking shall be done upon getting out of the Campus.

E.3 If a delivery of item is uncertain, consider it as hazardous, or toxic that


may cause harm to the physical or health status of the persons inside the
Campus or to the facilities therein. A Material Safety Data Sheet (MSDS)
document shall be generated and the Campus Safety Officer shall perform item
examination per OSH standards.

F. INTRUDER PROTOCOLS

If an individual or group of persons intrude the Campus, threatening to cause


or causing serious harm upon persons, it is highly recommended for security
personnel at the entrance gate to hold the ground and advise the persons
nearby to: (1) run away from the threat as quickly and safely as possible; (2)
not to run in a straight line; and (3) use buildings, trees, shrubs, cars, as cover.

G. ACTIVE SHOOTERS

In the event of possible active shooters that might disturb and harm the safety
and security of every person inside the Campus, especially while having
classroom instruction, it is highly recommended that a lockdown information
lecture and a drill must be done independently at regular intervals.

H. MANAGING LARGE CROWD-DRAWING EVENTS

H.1 Crowd management strategy shall be planned ahead of time. This


should be attended by the host committee, the event organizers, the CSSO, the
IMT Commander, PNP, BFP and other necessary parties as the event may
require.

H.2 In preparing for a large crowd-drawing event, an Incident Command


System (ICS) shall be established which shall comprise of medical personnel,
first aiders, rescuers, standby ambulance and fire truck and crews, PNP
uniformed personnel, and uniformed event security personnel of CSSO.

H.3 During the event, there must be “Event Marshalls” who shall actively
conduct “on-see” until the event has been done.

Section 2. EMERGENCY PLANS AND RESPONSE

1. FIRE EMERGENCY

a. BEFORE:
1. Identify and report fire hazards and unsafe work practices.
2. Unplug all electrical equipment when not in operation, unless
necessary.
3. Maintain office sanitation and proper housekeeping (apply the 5’S).
● SORT - sort out unnecessary items in the workplace and discard
them.
● SYSTEMATIZE - arrange necessary items in good order.
● SWEEP - clean your workplace completely.

● SELF-DISCIPLINE - train people to follow good work habits.
● STANDARDIZE - maintain one’s workplace so that it is
productive and comfortable.
4. Familiarize and keep emergency exits marked, lighted and free from
any obstruction.
5. Familiarize location of fire alarm, knowledge on how to use fire
extinguishers and other firefighting equipment.
6. Enforce “No Smoking” policy and report violators.
7. Welding work must have work permit (Hot work Permit – BFP) and
fire extinguishers at hand for the duration.
8. Proper observation of retention and disposition of
documents/records.

b. DURING:
1. After the first sign of fire, make every attempt to put it off. The first
three (3) minutes is critical as it is still containable with fire extinguishers.
Thereafter, the nearest fire alarm should be sounded.
2. If evacuation is ordered, stay calm. Don’t panic. Follow your Floor
Marshal and his assistant toward the emergency exit opposite of the fire
location, in an orderly fashion.
3. If your life is not threatened, make every effort to search your work
area to see any possible straggler and lead them to the exit stairwells
downs to the ground floor.
4. Do not use the elevator during evacuation.

c. AFTER:
1. Check yourself for any bodily injury and immediately report for first
aid treatment.
2. Report to your Floor Marshal for physical accounting at the
designated assembly area.
3. Report your colleague who is probably missing.

2. EARTHQUAKE EMERGENCY

a. BEFORE:
1. Determine if site is along an active fault or prone to liquefaction or
landslides.
(If you need help, consult PHIVOLCS).
2. Make sure that proper structural design and engineering practice is
followed when constructing a building.
3. Check the integrity of your buildings
● Secure structural plan of the building.
● Identify structural elements.
● Determine condition of each structural element.

4. Know all exit areas
5. Identify safe spots:
● Under a sturdy desk or table.
● In a strong supported doorway.
● In an inside corner of the room.
6. Identify danger zones:
● Glass windows and glass panels.
● Book shelves, machinery, cabinets and furniture that may topple
or slide.
● Any kind of hanging objects.
7. Prepare workplace:
● Secure all hanging and heavy objects.
● Store breakable items, harmful chemicals, and flammable
materials in lowermost shelves and secure.
● Keep heavy materials in lower shelves.
● Turn off gas tanks when not in use.
8. Earthquake Survival Kit:
● First Aid Kit (Inside pack bag) with Water Purification Tablets
● AM/FM Radio (Battery operated)
● Water & High-energy Food (Chocolates)
● Tools, Ropes & Knife
● Lighter, Match, Candles & Flashlights (Battery Operated)
● Blankets
● Tissue Paper & Waste Bags
● Pencil and Paper

b. DURING:
1. Secure an exit – When you feel an earthquake, open a door right
away. This ensures an escape route after the earthquake is over.
2. Do not rush outside – Usually a strong earthquake will end within a
minute or so. Seek shelter in the room you’re in and stay there until
it’s over.
3. Go to safe spots – Protect your head from falling debris.
4. If outside, move to an open area – Get away from power lines, posts,
walls, other structures that may fall or collapse. Stay away from
buildings with glass panes.
5. When driving a vehicle, pull to the side of the road and stop.

c. AFTER:
1. Take emergency kit.
2. Get out calmly in an orderly manner. Go to an open area.
3. Use the stairs. Do not use the elevator.
4. Check for and attend to injuries.
5. Check Surrounding.

● Check for fire and if any, have it controlled.
● Clean up chemical spills, toxic and flammable materials to avoid
any chain of unwanted events.
● Check water, gas and electrical lines for defects. If any damage
is suspected, turn the system off in the main valve or switch.
● Use flashlights; never light a match.
6. Help reduce number of casualties.
● Don’t enter partially damaged building, strong aftershocks may
cause further collapse.
● Gather information and disaster prevention instruction from
battery -operated radios.
● Obey public safety precautions/instructions.
● Limit telephone calls.
7. Document observation.
● Damaged parts of building
● Damaged utilities
● Fires
● School record/security logbook

3. BOMB THREAT AND EXPLOSION EMERGENCY

a. BEFORE:
1. Maintain good housekeeping.
2. Familiarize everything that normally belongs in your work area as to
those that do not.
3. Accost unescorted/suspicious person/s wandering in your work area
without legitimate purpose.
4. Considered abandoned packages/cars as suspect. Notify security
immediately. Never touch or move suspected items. Let the experts
handle it.

b. WHEN A THREAT CALL IS RECEIVED:


1. Take mental note of the exact “threat”, the voice and accent of the
suspect and the noise in the background.
2. Prolong the conversation by calmly talking to the suspect as to “where
the bomb is situated, how many minutes are left before it goes off,
what is the motive for such threat”, etc.
3. Immediately report to the CSSO.
4. When a search is ordered, see what belongs to your area and
otherwise.
5. During the search, security shall give special attention to the comfort
rooms and emergency stairwells as probable hiding place of
explosives.

c. WHEN EVACUATION IS ORDERED:
1. Follow your Floor and Assistant Floor Marshals toward the fore exit.
Stay calm and do not panic. Remember that excitement can cause
confusion and may aggravate the situation.
2. Take the fire exit opposite to where the suspected bomb is situated.
3. Proceed straight to the designated area for accounting and
consolidation.
4. Check self for any injury that necessitates first aid treatment.

d. IF THERE’S EXPLOSION:
1. HOTZONE
● Be calm and don’t panic.
● Evacuate to the safest area. Around 300 meters away from the
explosion site.
● Seek assistance (call for help) e.g. Police, Bomb disposal unit,
fire department, EMS, Rescue etc.
● Don’t drag or pull severely wounded victims to avoid further
injuries.
● Let the rescuers do the job of extricating the victims.
● If you are profusely bleeding but can move your hands, try to stop
the bleeding by applying direct pressure to the wound to prevent
further blood loss.
● Remember your first aid.
● Don’t use any electronic device such as cellular phones, pagers
or handheld radios etc. for they can trigger or activate secondary
explosions.

2. COLD ZONE
● Do not enter the 300-meter barrier.
● Follow the orders of the security personnel.
● If you are a first aider/MD/RN, look for the medical officer in
charge on site, make the proper introduction and offer additional
help if needed.

4. FIRE FIGHTING PROCEDURES

a. PRE-FIRE PLANNING
Includes the area mapping, water source, degree of combustibility. Fire
safety lectures is necessary and on-site fire drill should be done.

b. SIZE-UP
To determine the firefighting needs of the area. Consider wind directions.
Water supply. Human interventions among others.

c. RESCUE OPERATIONS
Removal of victim/s from place of danger to a safer place.

d. COVER EXPOSURE
To prevent the fire into extending/spreading to other uninvolved
buildings.

e. CONFINEMENT
To prevent the fire from spreading another partition or section of the
building or to adjacent building.

f. VENTILATION
It is the process of driving out the smoke inside the burning building for
fast firefighting operations.

g. EXTINGUISHMENT
The actual putting out of the fire by the use of pressured hose or fire
extinguisher.

h. SALVAGING
It is the protection of unburned materials or building and contents from
unnecessary damage due to water, smoke and other elements.

i. OVERHAUL
To complete the operation after the main body of fire has been
extinguished. The operation which are necessary to prevent the fire from
rekindling and to place the building in safe condition.

j. POST FIRE ANALYSIS


For investigation processes.

5. FLOOD EMERGENCY

a. BEFORE:
● Have disaster supplies on hand or keep them in shelter locations.
● Monitor local radio, television, internet and social media for the latest
information and updates.
● Activate alarm where applicable and announce evacuation.

NOTE: Be ready to evacuate at a moment's notice, know your routes and


destinations. Find a local emergency shelter. Know where to go and how
to get there should you need to get to higher ground, the highest level of
a building, or to evacuate. Authorities will direct you to leave if you are in

a low- lying area, or within the greatest potential path of the rising waters.
Flashfloods develop quickly. Do not wait until you see rising water.

b. DURING:
Staying Safe Indoors:
● Turn off the power and water mains if instructed to do so by local
authorities.
● Avoid contact with floodwater. It may be contaminated with sewage
or contain dangerous insects or animals.
● Continue to monitor local radio, television, internet and social media
for the latest information and updates.
● Don’t use gas or electrical equipment that has been flooded.
● Dispose of any food that comes into contact with flood water. As a
rule of thumb, do not eat any food that has come in contact with
floodwater.

Staying Safe Outdoors:


1. Don't walk or drive through floodwater. Just six inches of fast-flowing
water can knock you over and two feet will float a car.
2. If caught on a flooded road with rapidly rising waters, get out of the
car quickly and move to higher ground. Do not rely on cars or other
vehicles to protect you from floodwaters; people are most likely to
drown inside a vehicle.
3. Don’t allow children to play in or near flood water. Even shallow
standing waters holds hazards particularly for small children. Use a
pole or stick to make sure that the ground is still there before walking
through an area where the water is not flowing.
4. Avoid contact with floodwater. It may be contaminated with sewage
or contain dangerous insects or animals.
5. Stay out of areas subject to flooding. Underpasses, dips, low spots,
etc. can become filled with water.

c. AFTER:
● If evacuated, return only when authorities say it is safe to do so.
● Continue to monitor local radio, television, internet and social media
for the latest information and updates.
● Keep children and pets away from hazardous sites and floodwater.
● If power lines are down outside, do not step in puddles or standing
water. Report them immediately to the power company.
● Be aware of areas where floodwaters have receded and watch out
for debris. Floodwaters often erode roads and walkways.

Note: Keep in mind that any flooded area is prone to additional flooding.
Even a little rain can turn a once flooded area into a sudden flash flood.

Also, floodwaters may have swept debris and other hazardous materials
into an area. Look out for glass, downed power lines, ruptured gas lines,
damaged buildings, and so on. Floodwater itself can be contaminated by
gasoline, oil, sewage, or other chemicals.

Section 3. EMERGENCY EVACUATION AND RESCUE

A. EMERGENCY EVACUATION
1. The Incident Management Team (IMT) shall be alerted and dispatch staff
to Gym/Evacuation area according to specific Campus Evacuation Plan.
2. Safety and Security Chief shall be notified to report to the Gym/Evacuation
area, and in turn shall notify Administration and Maintenance personnel to
report to the Gym/Evacuation area. The assigned Admin Clerk shall take
100 pens and Reunification Forms (Crisis Box) to the Gym/Evacuation
area.
3. The IMT shall coordinate with the Transportation Team to request buses to
be dispatched to the Campus for the purpose of evacuation.
4. The need for evacuation shall be announced, and upon hearing the
announcement, Student Care Teams (for activation per classroom) and
school employees or personnel shall rapidly exit the building according to
the Campus Exit Plan and wait for further instruction to board a bus
(approximately 30-40 people per bus).
5. A Back-up IMT member shall ride the first bus and be in charge of students,
employees and personnel in the Gym/Evacuation area.
6. Buses shall unload at a designated entrance door to the Gym/Evacuation
area. Students shall wait in the Gymnasium/Evacuation area.
7. Parents/guardian are to park in the parking lot and enter through the
Gym/Evacuation area through the designated exit door. Head and
members of the Site Security Team shall direct traffic, if needed.
8. Step 1 (Main Entry)
a. Upon entering, the parents or guardian (if any) shall pick-up and
complete the “Parent/ Guardian Request” section of the Reunification
form. Once completed, parents/ guardian shall exit to the lobby to wait.
b. Entry will be staffed with designated IMT staff.
9. Step 2 (Exit Protocol)
a. Once identity process is through, the student may be released with
his/her parent/ guardian and tailed with IMT staff until outside the gate.
Students who are of legal age may be released upon determination that
the circumstances is already safe.
b. The gate personnel (exit guard) must have a copy of the reunification
form and have it logged before final release of the person/s indicated in
the said form.

B. EMERGENCY RESCUE

1. Emergency rescue is a procedure of moving a victim from a dangerous place
to a safe place.
2. The following are indications of emergency rescue: (a) danger of fire and
explosion; (b) danger of toxic gases or asphyxia; (c) serious traffic hazard;
(d) danger of electrocution; (e) danger of collapsing wall; (f) exposure to
intense weather; (g) risk of drowning.
3. The CSSO shall organize a Search and Rescue Team (SRT) who shall
undergo continuous training, and shall be responsible of performing search
and rescue procedures for the University in times of emergencies.

Section 4. PROHIBITION/ RESTRICTION OF FIREARMS

A. PURPOSE
To ensure that USTP maintains campuses to be safe and free from any form
of violence, the University prohibits the possession or use of firearms in the
University property, except uniformed government personnel of the
government who will enter the campus on official purpose.

B. SCOPE
All students, employees and personnel are subject to this provision, including
contract workers as well as visitors, except for a uniformed personnel with
official purpose to the Campus, and those that may be authorized by law or
regulation.

A license to own and possess firearm (LTOPF), or any similar instrument, does
not supersede this University policy. Holder of LTOPF or any similar
instrument, may be allowed entry to the Campus, but they shall surrender their
firearm at the CSSO. Uniformed personnel of the government who will enter
the Campus on official purpose may be allowed to carry his/her firearm.

Any employee, personnel, or student in violation of this policy shall be


subjected to appropriate disciplinary action. Visitors however, who are in
violation of this policy, shall not be allowed to enter the Campus premises.

C. SEARCHES OF PERSONAL PROPERTY


The USTP reserves the right at any time and at its discretion to conduct routine
or border search upon all University-owned or leased vehicles, and all
vehicles, packages, containers, briefcases, purses, lockers, desks,
enclosures, and any persons entering its property, for the purpose of
determining whether any firearm is being, or has been, brought onto its
property or premises in violation of this policy. Employees, personnel, or
students who fail or refuse to promptly permit a search under this policy shall
be subject to appropriate disciplinary action. Visitors, however, who fail or

refuse to promptly permit a search under this policy will not be allowed to enter
the Campus premises.

D. ENFORCEMENT
This policy is administered by the Office of the Vice President of Administration
and Legal Affairs (OVPALA) and enforced by the CSSO. Anyone with
questions or concerns specific to this policy should contact the OVPALA.

Section 5. REPORTING OF CRIME/ INCIDENTS AND INVESTIGATION

A. PURPOSE
USTP is obliged to report specific criminal incidents, occurring in or within the
Campus properties and facilities.

The University takes seriously the safety and well-being of all employees,
personnel, students and visitors. This policy is developed to provide a clear
and consistent process for reporting incidents that involves safety and security
of students, employees, personnel or visitors, and to establish an effective
means of communication of the incident to the proper personnel for
appropriate action, response, or documentation.

B. STATEMENTS
All employees, personnel, students, and visitors are requested to assist in
making the Campus a safe place by being alert to suspicious situations or
persons, and by reporting them as outlined below.

In the scenarios hereunder, when the CSSO is informed of an incident,


anonymity of the reporter, victim and accused shall be observed. Names and
details are to remain confidential pursuant to the Data Privacy Act.

Anyone who is a victim of, or a witnesses to, or is involved in any violation of


law such as assault, robbery, theft, or acts of sexual violence, or any similar
offense or crime that could pose a threat to the health and safety of any
person(s), that was committed in any of the Campus-owned or leased
property, or during any Campus sponsored activity, should contact the
Emergency National Office (911) and follow their directions. In addition, the
individual shall immediately report the incident to the CSSO which shall inform
other concerned offices, as applicable.

Offenses or incidents of a lesser nature including, but not limited to lesser


property crimes shall be immediately reported to the CSSO which shall inform
other concerned offices, as applicable.

If an individual notices any person acting suspiciously, he/she shall contact
immediately the CSSO or the Emergency National Office (911).

Incidents requiring medical attention should be reported to the Emergency


National Office (911). In addition, the individual should immediately report the
incident to the CSSO which shall inform other concerned offices, as applicable.
The CSSO shall execute an incident report.

This shall be posted in conspicuous places of the Campus.

C. TIMELY WARNINGS AND EMERGENCY NOTIFICATIONS


In the presence of an immediate threat to the health or safety of students,
employees or personnel occurring within the campus, USTP shall immediately
issue an emergency notification to the Campus community in the geographical
area affected by the emergency, upon confirmation of the emergency or
dangerous situation. The entire Campus community shall receive adequate
follow-up information as needed.

USTP shall provide information to the entire Campus community regarding


crimes that are considered by the institution to represent a threat to students
and employees. Notifications are considered timely warnings and are aimed
at the prevention of similar crimes from recurring.

In case of any emergency, the Office of the System President shall designate
an Official Spokesperson of the University (OSU) who shall be the sole
officer responsible of releasing any relevant information to, and to answer for
any queries or questions coming from, the community.

D. INVESTIGATION
For purposes of determining identity and location of the victims and offenders,
the Office of the System President may designate an investigator who shall be

responsible of investigating into the circumstances surrounding the offense or
crime, particularly the identity and location of the victims and offenders, and to
gather evidence to prove the guilt of the offender for any possible legal action
by the University.

If warranted under the circumstances, local law enforcement investigators may


be invited to collaborate in the investigation.

If the offense or crime is against any person not connected with or property not
owned by, the University, filing of appropriate case in a court of law may be
resorted to by the aggrieved party. If the University is the offended party, filing
of any case in the court of law shall be with the approval of the Board of
Regents.

Section 6. RESPONSE TO THREATS


A. USTP shall organize in each of the campuses, a Threat Assessment Team
(TAT) which shall be tasked to convene to identify, evaluate and address
threats or potential threat to the Campus security.
B. It shall also be tasked to review incidents of threatening behavior by students
(current or former), parents, school employee or other individuals.
C. Members of the TAT shall be the campus chancellor/administrator/director,
guidance counsellors, CSSO officials, as well as outside medical and mental
health professionals and local law enforcement officers.

Section 7. VEHICLE TRAFFIC AND PARKING


A. Employees, personnel, students and visitors driving a vehicle, motorcycle or
bicycle may enter the school premises; provided, he/she shall submit himself/
herself to the security and safety measures of the Campus. Subject to
reasonable amount of parking fee to defray administrative costs, vehicle gate
pass may be issued to any vehicle that may enter the Campus.
B. At the vehicle inspection gate, the duty security personnel may ask the visitor-
driver to open the trunk of the car for visual inspection, and close the same after
the inner and the under chassis were viewed around by a crocodile-mirror
security tool.
C. Drivers may park their vehicle inside the Campus on a first-come-first serve
basis, except for parking space expressly reserved for University/Campus
officials, under the direction of the parking security personnel and visible
signage. The University reserves its right to charge reasonable amount of
parking fee to defray administrative costs. In case of collision or swipe accident,
an investigation shall be made with the assistance of roving security personnel
and CCTV footage on the ground.

D. All drivers are advised to comply with the school-zone speed limit of (20 km/h),
and to avoid overtaking and blowing of horn as it may destruct ongoing classes.

Section 8. CAMPUS BUS/ SHUTTLE SECURITY AND SAFETY

A. RULES AND REGULATIONS


USTP ensures that passengers experience a safe, secure and comfortable
ride by enforcing Campus bus/shuttle security and safety rules and
regulations. Passengers who fail to follow the rules and regulations or
bus/shuttle drivers instructions may be removed from the bus/ shuttle or denied
bus/ shuttle service.

For obvious reasons, weapons and illegal substances are not allowed in any
bus/ shuttle. In addition, smoking, foods and drinks are not permitted on
bus/shuttle services.

Waste receptacles are located at the front of each bus/shuttle and passengers
are asked to help keep the bus/shuttle clean and litter free.

For the security and safety of all the passengers, some additional items are
restricted or prohibited in USTP bus/ shuttle. All items being transported on the
bus/shuttle are allowed at the sole discretion of the driver of the bus. Other
items not permitted on board are gasoline or flammable liquids, toboggans or
sleds, furniture or mattresses, large pieces of lumber, open alcoholic
beverages, car batteries or weapons of any kind.

The front seats are reserved for seniors and passengers with mobility
impairments. Remember to keep the isle clear. Passengers are encouraged
to be courteous of fellow passengers, to speak softly, to not use abusive
language and to watch noise levels when using cell phone, or other electronic
devices requiring headphones.

B. AUDIO AND VIDEO SURVEILLANCE


All University bus/shuttle may be equipped with audio/video security
equipment. This equipment may be turned- on at all times while the vehicle is
running. Vehicles equipped with security equipment include signs or decals
posted in various locations on the interior of the vehicle to advice passengers
of the presence of such equipment. Access to recordings made by this
equipment is limited to the CSSO and may be used for the following purposes:
● Incident or accident investigation
● Maintenance
● Random checks to ensure staff compliance with laws, policies,
procedures, and other regulations

When needed, such recordings may be disclosed to law enforcement agencies


to be used as evidence in their investigations.

Section 9. EPIDEMIC, PANDEMIC AND PUBLIC HEALTH CONCERNS
A. In coordination with the Campus Health Officer (CHO) or its equivalent, strict
implementation of safety or health protocols established by a duly organized
government body shall be done at pedestrian and vehicle inspection gate for
all employees, students and visitors entering the Campus.
1. Where, appropriate, the following may be required upon entry:
● Sanitize hands before entering
● Wearing of face mask
● Temperature screening
● Closely monitor persons with high temperature for a few minutes.
● If temperature does not drop, CSSO personnel shall immediately
escort employee/student to the Medical Clinic. Visitors may be
advised to not enter the Campus, or have him be checked with a
competent external medical practitioner.
2. Record in logbook all entries into the Campus by visitors.
B. Regular inspection and monitoring of all buildings and areas if minimum health
protocols are to be observed at all times.
1. Safety Marshals shall be assigned in each building/areas.
2. Constant reminders for all employees, students and visitors to follow
minimum health standards (e.g., wearing of face mask & social
distancing).
3. Make a report of employees/students not following safety protocols.
4. Close coordination with Medical Unit and other concerned offices
regarding safety protocols.
C. Report of any cases of illness inside the Campus premises. CSSO personnel
shall immediately escort to the Medical Clinic any employees/students who are
apparently suffering illness.

Section 10. CAMPUS RALLY, DEMONSTRATION, OR OTHER FORMS OF MASS


OR CONCERTED ACTION

A. GENERAL STATEMENT
Discussion and expression of views is encouraged and permitted within
USTP, subject only to requirements for the maintenance of safety and order.
Support for any cause by orderly means, which is not in violation of law, and
which does not disrupt the operation of the University, nor interfere with the
rights of others, is permitted. USTP retains the right to ensure the safety of
individuals, regulate the time, place, and manner of demonstrations, the
protection of property, and the continuity of the educational processes.

B. OBTAINING A WRITTEN PERMIT FROM THE LOCAL MAYOR

USTP Students, employees, personnel or a combination thereof, who wish to
schedule any rally, demonstration, march, parade, procession or any other
form of mass or concerted action to be held within any public place in the
Campus, for the purpose of presenting a lawful cause, or expressing an
opinion to the general public on any particular issue, or protesting or
influencing any state of affairs whether political, economic or social, or
petitioning the government for redress of grievances, shall comply with the
provisions of Batas Pambansa Bilang 880 (BP 880), otherwise known as “The
Public Assembly Act of 1985”.

Pursuant to the same law, any rally, demonstration, march, parade, procession
or any other form of mass or concerted action for the purpose as above-
mentioned, to be held within any of place of the USTP Campuses, shall only
proceed with after the issuance by the concerned local mayor of a written
permit for such activity.

Prior to the actual holding of the activity, the same written permit shall be
presented to the System President or Campus Chancellor or Campus
Administrator or Campus Director, where the rally or demonstration will be
done.

In turn, the office of the System President or Campus Chancellor or Campus


Administrator or Campus Director shall inform the CSSO which shall prepare
for the event in coordination with the nearest local law enforcement agencies.

C. PROCEDURES FOR HANDLING DISRUPTIVE DEMONSTRATIONS OR


SIMILAR ACTIVITIES
In situations where there is a group demonstration, the CSSO, in consultation
with the VPALA/or the Office of the Student Affairs (OSA), shall determine the
point at which a demonstration or similar activity becomes disruptive based
upon the criteria set forth in BP 880.

If the situation permits, the CSSO, the VPALA, or the OSA, or a designee shall
inform the demonstrators that they must discontinue their disruptive activities,
explaining which activities are in violation of this Procedure, and shall advise
them how to continue their demonstration in a manner which is not disruptive.
If the disruption continues, appropriate action shall be taken, which may
include police or local authority action. An immediate physical threat to persons
and/or property may require immediate police action, as well as situations in
which the VPALA, or the OSA, shall determine that a warning will not provide
adequate protection for persons or property.

D. CONSEQUENCES OF VIOLATING THE POLICY

Any violation of this Policy shall be proceeded against pursuant to the penalties
provided for under BP 880 and all other applicable laws on the matter.

Section 11. ALCOHOL AND DRUGS

A. PURPOSE
In compliance to DOLE Department Order No. 53-03 entitled “Guidelines for
the Implementation of a Drug-Free Workplace Policies and Programs”, USTP
has a longstanding commitment to provide a safe, quality-oriented and
productive work environment. Alcohol and drug misuse poses a threat to the
health and safety of USTP employees, students, and to the security of the
University's equipment and facilities. For these reasons, USTP is committed
to the elimination of drug and alcohol use and misuse in the workplace.

Employees should report to work fit for duty and free from any adverse effects
of illegal drugs or alcohol. This policy does not prohibit employees from the
lawful use and possession of prescribed medications. Employees must,
however, consult with their doctors about the medications' effect on their
fitness for duty and ability to work safely, and they must promptly disclose any
work restrictions to their supervisor.

B. SCOPE
This policy applies to all employees, personnel, students, and visitors of USTP.

C. WORK RULES
1. Whenever employees are working, are operating any USTP vehicle, are
present on Campus premises during work-hours, or are conducting
University-related work offsite, they are prohibited from:
● Using, possessing, buying, selling, manufacturing or dispensing an
illegal drug, to include possession of drug paraphernalia.
● Being under the influence of alcohol or any illegal drug.
● Possessing or consuming alcohol.
2. The presence of any detectable amount of any illegal drug, illegal
controlled substance or alcohol in an employee's body system, while
performing their duties or while in a campus facility during work-hours, is
prohibited.
3. USTP shall not allow employees to perform their duties while taking
prescribed drugs that are adversely affecting their ability to perform their
duties safely and effectively. Employees taking a prescribed medication
must carry it in a container with prescription issued by a duly licensed
physician.
4. Any illegal drugs or drug paraphernalia shall be turned over to any
appropriate law enforcement agency which may result in criminal
prosecution.

D. CONSEQUENCES OF VIOLATING THE POLICY
Any violation of this Policy shall be proceeded against pursuant to the penalties
provided for under Republic Act No. 9165, as amended, and all other
applicable laws on the matter without prejudice to any administrative or
disciplinary proceedings that may be instituted against the erring student,
employee, or personnel.

Section 12. VIOLENT OFFENSES


A. USTP is committed to the safety and security of all persons. To ensure a safe
workplace and to reduce the risk of violence, all employees should review and
understand all provisions of this workplace threats and violence policy.

B. THREATS AND VIOLENCE


The University will not tolerate any threats, threatening behavior, or acts of
violence committed by or against any employee or personnel or student within
the University property. Violations of this policy shall lead to disciplinary action
up to and including dismissal, as well as arrest and prosecution for any criminal
acts.

C. PROHIBITED CONDUCT
Employees, personnel or students are prohibited from making threats or
engaging in violent activities. This list of behaviors, while not exhaustive,
provides examples of conduct that is prohibited, to wit:
i. Causing physical injury to another person.
ii. Making threats of any kind.
iii. Aggressive, hostile or violent behavior, such as intimidation of others;
attempts to instill fear in others; or subjecting others to emotional
distress.
iv. Other behavior which suggests a propensity toward violence, which may
include belligerent speech, excessive arguing or swearing, sabotage or
threats of sabotage of University property, or a demonstrated pattern of
refusing to follow University policies and procedures.
v. Intentionally damaging University property or property of another
employee, personnel, or student, or member of the public.
vi. Possession of a weapon while in a University property or while in
University business, with the exceptions indicated in the firearms policy.
vii. Committing acts motivated by, or related to, sexual harassment or
domestic violence.

D. DEALING WITH CONFRONTATION


Employees who confront or encounter an armed or dangerous person should
not attempt to challenge or disarm the individual. Employees should remain
calm, make constant eye contact and talk to the individual. If a supervisor can

be safely notified of the need for assistance without endangering the safety of
the employee or others, such notice should be given. Otherwise, cooperate and
follow the instructions given.

E. REPORTING
University employees or personnel are responsible to notify the CSSO of any
threats they may have witnessed or received, or any behavior they have
witnessed which they regard as threatening or violent, when the threat or
behavior is job-related, or might be carried out in University property, or is
connected to University employment.
a. Reports can be made anonymously and all reported incidents shall be
investigated.
b. Reports or incidents warranting confidentiality shall be handled
appropriately and information shall be disclosed to others only on a
need-to-know basis.
c. Employees who have obtained restraining orders or personal protection
orders, which include the University campus as a protected area, should
immediately provide a copy of the order to the CSSO.

F. ENFORCEMENT AND DISCIPLINE


Upon notification to the CSSO, the person making threats, exhibiting
threatening conduct, or committing any other acts of aggression or violence in
the University shall be removed from the University property as quickly as
possible, and shall remain off the University property upon pending
investigation. Notification of the nearest law enforcement authorities shall be
done for assistance. Any employee or student determined to have committed
such acts will be subject to disciplinary action, up to and including termination
and/or criminal prosecution. Non-employees engaged in violent acts in the
University property shall be reported to the proper authorities and be fully
prosecuted.

Section 13. PERSONAL SECURITY FOR ADMINISTRATORS


a. Personal security of every school administrator is essential. This is to deter,
delay and/ or provide warning of any possible crime; if such warning occurs,
to summon assistance and prepare for the possibility of crime in a constructive
manner.
b. As may be necessary, a bodyguard/ protection agent shall be detailed to the
University President or Chancellors or any officials of the University.
c. The detailed bodyguard/ protection agent shall be required to train on the
principles and practice of protective security formation, site and route survey,
walking formation, among others.

Section 14. SECURITY AND SAFETY MANAGEMENT
A. This is a process of USTP that uses procedures and actions to prevent or
reduce the chances of experiencing harm or loss of lives and properties. The
entirety of this policy is the Continuous Quality Improvement (CQI) of USTP
in terms of security and safety.

B. ROLES OF SECURITY PERSONNEL IN USTP


a. The main task of the security guard is to protect the university constituents
within the campus. They are there to protect and police the students or
any personnel with respect to the university policies and guidelines.
Compliance to such policies and guidelines is the joint responsibility of the
security guard and the respective heads of offices. The job also requires
a good deal of interaction with the students, employees, personnel, or
staff, as well as visitors and officials. Therefore, utmost respect and dignity
must be upheld during any interaction with all university stakeholders, and
always with a view towards their safety and security.
b. In some situations, school guards may be required to use reasonable
physical force against attackers. Because of not being armed, one may
want to be knowledgeable of some form of self-defense. One may
consider himself ready in responding to fights between students and
emergency situations.
c. School security personnel may also be required to know the basic on first-
aid, firefighting and some rescue practices and procedures.
d. Security personnel need to be an individual that can stay calm under
pressure. Students can be very unruly at times and so can some parents
and stakeholders. Being calm under pressure, instead of erupting, may
help a security personnel successfully deal with some situations without
getting physical.
e. The security personnel should set patrol posts at the Campus’ surrounding
to build a security network and to maintain the protection level.
f. The security personnel should supervise student’s activities by intervening
movement that may result to injury. Security personnel should patrol the
dormitory and the whole Campus from time to time.
g. Serve as a role model for student’s behavior, attire, and professional
grooming.
h. Help supervise arrival and dismissal of students as well as breakfast and
lunch periods.
i. Help students to get to class on-time and ensure a smooth flow of traffic
through the hallways.
j. Intercept visitors and check for proper identification and escort to exits if
not authorized.
k. Notify police and emergency personnel when there is emergency or
unusual situations.

l. Notify the appropriate office when there is evidence of substance abuse
severe medical conditions, child abuse, potential suicide, alcohol abuse,
etc.
m. Assist police, fire personnel, and other emergency crews when necessary.
n. Assist in the conduct of emergency response, feedback and reports from
employees and clients.
o. Maintain alert and kin while on tour of duty especially to danger and to
something that is wrong.
p. Inspect the bringing out and returning of properties when used outside.

D. ORGANIZATIONAL STRUCTURE OF USTP SECURITY AND SAFETY

Objectives of the Organizational Structure

a. Streamline the organization of Security and Safety who shall enforce all policies,
rules, and regulations relative to the protection of lives and properties of all the
stakeholders of the university.

b. Creation of a security group for the University President and Chancellors.

c. Establish a definite line of command and authority from the President down to the
security services and safety officers.

d. Establish a definite line of command from VCFAs of the main and major campuses
down to the security services and safety officers.

e. Establish a definite line of command from the directors of the satellite campuses
down to the security services and safety officers.

Implementing Guidelines

a. The determination of the necessity to activate a Presidential Security in the


President’s office or Chancellors’ Security in the Chancellor’s office rests jointly in
the President or Chancellor and with the Security Council.
b. The Security Council shall be composed of all the Security and Safety Chiefs of all
campuses of USTP and the Vice President for Administration and Legal Affairs. If
the President or Chancellor and the Security Council see the necessity, then it may
activate the same security group which may be composed of job order or agency
guards or plantilla, to a maximum of three (3) personnel. The necessary budget
accompanying such creation shall be included in the OPCR of the President and
Chancellor’s offices.
c. The Security Council shall recommend a team leader who shall be designated by
the President or the concerned Chancellor. The team leader shall exercise
command, supervision and control over the security group in the President’s office
or Chancellor’s office. The President or the Chancellor shall exercise command,
supervision, and control over the team leader of the security group.

d. The Security and Safety Chief of each of the campuses shall be a regular or
plantilla personnel who may be appointed or designated into the same position.
e. Each of the campuses of USTP shall appoint or designate a Chief of Security and
Safety who shall be under the supervision and control of the VCFAs in case of the
main or major campuses, or under the supervision and control of the Campus
Directors in case of satellite campuses.
f. The Chief of Security and Safety shall exercise command, supervision and control
over all security and safety personnel under its campus.
g. All of the Chief of Security and Safety shall coordinate with the Security and Safety
Coordinator of the USTP System for purposes of policy and organization concerns.
h. The Security and Safety Coordinator of the USTP System shall be under the
supervision and control of the Vice President for Administration and Legal Affairs.
i. The Security Services of each campus shall be composed of a combination of
plantilla personnel and agency guards. The job order security guards shall be
abolished upon approval by the Board of this policy and upon successful
procurement of agency security guards.
j. The Security Officer and the Safety Officer of each of the campuses may be
appointed or designated into the same position.

k. When necessary, the principles in law enforcement or security organization shall


be exercised such as, but not limited to: (1) division of labor, (2) unity of command,
(3) chain of command, (4) delegation of authority and responsibility, (5) delineation
of authority and responsibility, and, (6) span of control.

PART THREE.
IMPLEMENTATION, AMENDMENT, EFFECTIVITY AND APPENDICES

Section 1. PERIODIC AMENDMENTS


This Security Policies shall be subject to periodic amendments that may be
initiated by any of the policymakers of USTP. Provided, that no amendments
shall take effect without approval by the Board of Regents.

Section 2. REPEALING CLAUSE


All issuances which are contrary or inconsistent with this Policy are hereby
repealed and/ or modified accordingly.

Section 3. SEPARABILITY CLAUSE


If any provision of this Policy or application of such provision to any circumstance,
is declared invalid or unconstitutional, the other provisions not affected thereby
shall remain valid and subsisting.

Section 4. EFFECTIVITY
This Security Policy shall take effect upon approval by the Board of Regents.

2. Design the annual safety programs and measures and plans for action in cases
of emergency.
3. Monitors security situation in the locality and keeps the Fire Marshal properly
informed.
4. Schedule activities promoting safety awareness.
5. Responsible in the operating condition of all firefighting equipment in the
building at all times.

c. ALARM ANDCOMMUNICATION TEAM


1. Takes charge in sounding the alarm when fire or emergency situation occurs and
immediately call for assistance from agencies concerned.
2. Notify all brigade members, call fire stations and related disaster response
groups.
3. Organize the communication group to further monitor the situation and for further
instruction and public safety precautions.
4. Know the location of the fire alarm if fire strikes.
5. Learn the sound of your building’s fire alarm – 1 long alarm sounds means a fire
is in progress.
6. Use any audible communication device (whistle, bell etc.)

d. FIRE FIGHTING TEAM

ROLES AND RESPONSIBILITIES


1. Keep abreast to the Department of Environment and Natural Resources (DENR)
requirement as well as the USTP’s on pollution control.
2. Safety responsibilities in performing job in his/her area of influence.
3. Attend to the necessary permit requirement of the government (DENR) prior to operating a
facility.
4. Ensure compliance of Pollution Control Facilities to government standards.
5. On time submission of Quarterly Self-Monitoring Repost to DENR.
6. Act as liaison officer to DENR, USTP and other pollution regulating body including the local
government pollution control office.
7. Attend the meeting of pollution control officers which from time to time be called by DENR.
8. Monitor the generation and disposal of used oil, batteries and other Hazardous waste.
9. Keep records/document of hazardous waste disposal, manifest form and other related
supporting documents.
10. Handle other environmental concerns as required by the USTP.

4. BUILDING ADMINISTRATORS (MAYBE ASSIGNED WITH THE AMSD)

GENERAL OBJECTIVES
1. To help the University Administration in its overall efforts to bring down the cost of electricity,
water and telephone/cell phone services.
2. To achieve and maintain safety and cleanliness in all buildings and facilities of the University.
3. To oversee the safety of building occupants, ensuring emergency exits are passable and
signage are visible and building is free of hazard.
GENERAL DUTIES AND FUNCTIONS OF A BUILDING ADMINISTRATOR
1. Ensure proper and judicious usage of all utilities of the University electricity, water, and
telephone specially those within his jurisdiction.
2. Check and monitor the physical condition, general upkeep and cleanliness of the building,
its surroundings and its facilities.

3. To coordinate with custodial workers, personnel of the Building & Ground Maintenance
Section (BGMS) and of such other pertinent offices necessary for the effective performance
of their duties and responsibilities.
4. Offer suggestions and/or recommendations to the Auxiliary Management Services Division
(AMSD) on matters affecting building administration and utilities management.

ON WATER FACILITIES
1. Leaking faucets and/or busted water pipes should be reported immediately to the Auxiliary
Management Services Division (AMSD) for immediate action.
2. In areas where water supply is not utilized during the night the main water gate valve of the
building should be shut off at 6:00 PM and reopened at 6:00 AM the next day.
3. Use of University water system for working private vehicles should be PROHIBITED.
Similarly, washing of clothes should not be allowed. Gardener must not leave their water
hoses running continuously for hours just to water a small garden plot or there’s pouring
rains.

ON ELECTRICAL FACILITIES
1. Electrical problems should be reported immediately to the Mechanical & Electrical Works
Section (MEWS) for immediate action. Such problems may be defective lighting facilities
(e.g., busted electric bulbs, malfunctioning fluorescent lights), open or dangling wires,
electric fans which are not properly functioning, etc.
2. After office hours, ocular inspection of each and every room should be conducted to see to
it that all lighting facilities which are not use are switched off. When electrical facilities are
not switched off after occupants have left and locked the room, the main switch may be
switched off, if power supply for the building will not be adversely affected. Security lights
should be put on/off on appropriate hours.
3. Use of electricity by non-USTP users e.g., dry paper copiers should be monitored. See to it
that only authorized equipment are plugged into the unit's electrical outlets.

ON AIR-CONDITIONING AND OTHER ELECTRICAL APPLIANCES / EQUIPMENT


1. Defective air conditioning units, including those that are running but not in good condition
should be reported immediately to the Mechanical & Electrical Works Section (MEWS).
Review periodically the service tags of each air conditioning unit within your jurisdiction and
see to it that regular checkups by pertinent MEWS personnel are carried out regularly.
2. Check other electrical appliances and equipment to ensure that they are properly
functioning.
3. University guidelines on the use of air conditioners and other electrical appliances should
be implemented. Report to the Dean, offices or individuals which do not follow any of the
guidelines.

ON TELEPHONE/ CELLPHONE FACILITIES


1. Report all PABX telephone troubles immediately to the Information and Communications
Technology (ICT) Office for proper action.
2. Report all PLDT phone troubles directly to PLDT for proper action and to be able to avail of
the payment deductions due to defective telephone units. Request for the (employee)
number of the person.
3. All areas within his jurisdiction, in and out of the building, should be regularly inspected to
see to it that the assigned custodial workers are doing their job properly. Delinquent custodial
workers should be reported directly to the Dean/Head of unit for proper action.
4. Waste receptacles should be requisitioned immediately for areas where they are needed or
lacking.

OVPALA.

ACCESSIBILITY OF THE BUILDING ADMINISTRATOR


1. The Building Administrator must be available during office hours and accessible even after
office hours. His/her telephone number, if any, and his/her address must be on file at the
administrative office of his/her unit.

BUILDING ADMINISTRATOR SPECIFIC DUTIES AND RESPONSIBILITIES:


1. Maintaining a safe environment with proper lighting, signage, and disability access.
2. Drafting and updating emergency plans and evacuation procedures.
3. Overseeing security, fire prevention, and other safety systems.
4. Scheduling regular building maintenance and janitorial services.
5. Contracting professionals for repairs as needed.
6. Ensuring that occupants are provided with proper utilities.
7. Monitoring building maintenance budgets.
8. Arranging for building improvements.
9. Overseeing contractors and inspecting completed jobs.
10. Supervising grounds staff.
11. Recommend for hiring of personnel or contractors as needed to maintain, repair, or
improve the property.
12. Providing training for building employees as needed.
13. Scheduling contractors and employees.
14. Resolving complaints, problems, and requests from building occupants.
15. Assisting with emergency response and evacuations.
16. Inspecting the building frequently for signs of damage or wear.
17. Maintaining records of building and its tenants/occupants (building permit, occupancy
permit, business permit, lease contract, personnel, etc.).
18. They shall also keep an updated record of regular inspection, scheduled testing and/or
regular preventive maintenance of all Fire Safety Features to ensure proper operational
condition.

5. INCIDENT COMMAND SYSTEM


Incident Command System is the model tool for command, control and coordination of a response
and provides a means to coordinate the efforts of individual agencies as they work toward the
common goal of stabilizing the incident and protecting life, property, and the environment.

➢ The ICS is a standardized approach to the command, control, and coordination of Emergency
Response providing a common hierarchy within which responders from multiple agencies can be
effective.
➢ Establishment of Incident Command Post (ICP) with various emergency Services at the Site.
➢ The ICP shall be established at the site of the incident and shall be under the control of the 1st
official to respond & arrive thereat. The first responders to emergencies are people near the affected
community & neighboring barangays.
➢ The Incident Commander (IC) shall initiate the proper mobilization of the various responders
who are operating in the area.
➢ The IC shall be directly in command & control of various emergency services.
➢ ICS has been proven effective for responding to all types of incidents, including:
● Hazardous materials (hazmat incidents);
● PLANNED EVENTS (e.g. celebrations, parades, concerts, official visit, etc.)

➢ Conduct collaborative planning projects for hazard identification, risk assessment and
vulnerability analysis as well as the conditions and circumstances under which those
risks might become potential threat to the University operations if not carefully managed;
➢ Institute measures to prevent/minimize accidents and/or cushion the impact of accidents
or disasters;
➢ Formulate incident management plans for each emergency situation, specifying the
course of action to be taken in case of emergencies.
➢ Organize Emergency Response Teams to handle specific emergency;
➢ Conduct periodic drills, training and exercises for the enhancement of response
capabilities.

11. Emergency Phase


➢ Assess the situation and choose the appropriate response and employ the Disaster
Control Groups;
➢ Update information on calamity and/or disaster through bulletins, advisories, and
forecasts from agencies concerned;
➢ Undertake immediate measures to prevent loss of life or serious property damage and
allow for flexibility in implementing the plan;
➢ Document every action taken during the response that will provide a record of appropriate

2. INCIDENT COMMANDER
● Assigned as the Commander of the Incident Management Team, the Incident Commander
must assume extra responsibilities. A decision should be made whether these responsibilities
are shared all the time or taken in turn (in some jurisdictions, the method of chairing meetings
is legislated).
● Specific Incident Commander duties may include:
➢ Scheduling meetings, notifying members
➢ Preparing an agenda
➢ Presiding over meeting
➢ Ensuring that the Team carries out its function
● Performing command activities, such as establishing command & establishing the ICP;
● Protecting life & property;
● Controlling personnel & equipment resources;
● Maintaining accountability for responder & public safety, as well as or task accomplishment
and;
● Establishing & maintaining an effective liaison w/ outside agencies & organizations, including
the EOC, when it is activated;
● Will be the senior-first responder to arrive at the scene. As additional responders arrive,
command will transfer on the basis of who has the primary authority for over-all control of the
incident. As the incident grow in size or become more complex, the responsible person who
has the jurisdiction in the area or agency may assign a more highly qualified Incident
Commander. After transfer of command, the outgoing Incident Commander must give the
incoming Incident Commander a full briefing & notify all staff of the change in command.
● Responsible for announcing evacuation and the emergency response procedure through the
use of public address system. If the public address system is disabled, he will use a
Megaphone or other portable means.

● WHO SHOULD BE THE INCIDENT COMMANDER (IC)?


➢ For rapid/slow onset natural disaster, the IC will be the local Chief Executive (LCE) or
duly designated officials depending on geographic location/jurisdiction.
➢ For human-made/technological disaster, the team leader of first responders arrives first
on the scene will be the Incident Commander.
➢ Transfer of command will be triggered when there’s a threat to life, environmental &
properties:
o Chemical/Hazmat: BFP (IN LAND) & PCG
o Combination of bombing & Hazmat: PNP
o Biological Epidemics: Dept. of Health
o Bombing/Explosion: PNP

3. PLANNING SECTION CHIEF


● The Planning Section Chief is responsible for assisting the Incident Commander with the

management of all academic and non-academic units.
● Gather, analyze, and disseminate information and intelligence.
● Maintain accountability for University personnel and account for personnel involved in or
affected by a campus emergency.
● Develop an Incident Action Plan.

4. SAFETY OFFICER
● The Safety Officer, with the help of the Incident Management Team, shall be responsible for
rallying all support departments into action in the event of an emergency.
● Identifies and evaluates safety and health hazards that may impact both the response workers
and the public and designates exclusion zone boundaries.
● Determines levels of personal protective equipment required
● Provides and/or coordinates health and safety training and regular safety briefings.

5. FINANCE/ADMIN SECTION CHIEF


● The Finance/Admin Section Chief supervises the Time, Procurement, Compensation/Claims,
and Cost Units, and is responsible for all financial and cost analysis aspects of the incident.

6. LIAISON OFFICER
● The Liaison Officer provides critical coordination and support to emergency preparedness and
response.
● Coordinates activities of the Incident Management Team with local officials.
● Maintains a log of all contacts made with regulatory/government agencies and identifies
representatives from each agency, including communication links and locations.

7. PUBLIC INFORMATION OFFICER


● The Public Information Officer makes sure that when a significant emergency occurs, he or
she provides accurate and timely information to the media and Emergency Authorities so that
the public remains informed about the scope and nature of the situation.
● The Public Information Officer serves as the primary spokesperson for the Incident
Management Team during incidents.

8. LOGISTICS SECTION CHIEF


● The Logistics Section Chief is responsible for obtaining and delivering resources requested
by the Incident Management Team, as well as providing all service and support functions for
the incident.

9. OPERATIONS SECTION CHIEF


● The Operations Section Chief is responsible for the management of all operations and
activities directly applicable to the primary mission of the Incident Management Team.

10. FIRST AID MEDICAL TEAM


● The First Aid Medical Team is responsible for ensuring that first aid supplies are available and
that first and triage is rendered during an emergency.
● Manage any and all medical situations depending on the severity.
● Activate the ambulance when it is necessary.
● The First Aid Medical Team Leader shall be responsible for providing an on-going status
report to the Safety Officer.

11. SITE SECURITY TEAM
● The Site Security Team is responsible for the security of the school site and its population
during an emergency.
● Ensure that exit/passage way is free of blockage.
● Will ensure that no one shall use their car to evacuate so as not to block the passageways.
● Responsible for the inspection of the entire facility to check for safety following an emergency
incident.
● Undertake safety and security inspections regularly and provide copy of report to the Safety
Officer.
● The Security Officer shall be responsible for providing an on-going status report to the Site
Security Team Leader.

12. SEARCH AND RESCUE TEAM


● The Search and Rescue Team is responsible for preparing and performing search and rescue
operations during an emergency.

13. FIRE BRIGADE TEAM


● The Fire Safety/Brigade Team is responsible for extinguishing fires and evaluating the
potential release of chemicals during an emergency.
● Responsible for evaluating the damages to school property in an emergency.

14. EVACUATION TEAM


● The Evacuation Team is responsible for the safe evacuation and accounting of all students
and staff during an emergency.
● Responsible for reporting missing persons to the Operations.
● Will relay reports of missing students to the Search and Rescue Team.

15. FLOOR MARSHALS


● Responsible for the immediate implementation of evacuation procedures upon the
announcement of the evacuation.
● Encourage all to remain calm and manage an orderly evacuation, avoiding the possibility of a
stampede. They must maintain composure but firm in implementing the 5 evacuation rules to
the evacuees – Don’t talk! Don’t push! Don’t run! Don’t turn back! Walk fast!
● Be visible in the scope of their respective section / area and lead employees and students out
through pre-determined safe exits towards the corresponding evacuation assembly areas.
● Instruct evacuees to stash their valuables in their hand carry bag and bring it with them. Leave
all other items/belongings behind. Ask ladies to remove their high heel shoes.
● Maintain order at the evacuation assembly area and check for injured persons and call for
First Aid to care for them.

16. MAINTENANCE TEAM


● The Maintenance Team is responsible for operating and modifying building systems and
utilities.
● Advising the Control Group of facility limitations and vulnerabilities.
● Securing floor plans of facility.

There is only one base per incident. (Incident name or other designator will be added to the
term Base). The ICP may be located with the base.

6. THREAT ASSESSMENT TEAMS


The primary component of a threat assessment program is a multi-disciplinary threat assessment
and management team. Involving members from an array of disciplines enhances the team’s ability
to:
➢ Identify developing concerns/threats.
➢ Gather information from multiple sources and organizational silos.
➢ Maximize skills and resources to address concerns.
➢ Monitor outcomes.
➢ Enhance the University’s overall ability to:
• Communicate (to, from, and within the team).
• Collaborate (working together for the best awareness and outcomes).
• Coordinate (engaging in purposeful planning and coordination of actions and
interventions within the team and with outside partners).

TEAMS ROLES AND RESPONSIBILITIES


Each member of a school threat assessment team should be aware of his or her role and
responsibilities. Roles and responsibilities for school threat assessment teams typically
include:
1. CAMPUS CHANCELLOR/ ADMINISTRATOR/ DIRECTOR
a. Consults with Team members to screen cases and determine when to conduct
an initial screening versus mobilize a full threat assessment inquiry.
b. Assists in conducting interviews of subjects, targets, witnesses, teachers, staff,
parents, and students.
c. Assists in gathering additional information (e.g., school records).
d. Determines and enforces disciplinary consequences, if appropriate.

e. Ensures that any threat management plan is followed and monitored.
f. Works closely with the public information officer or communications director to
respond to community concerns and questions.
2. SCHOOL PSYCHOLOGIST/ SOCIAL WORKER/ COUNSELOR
a. Assists in conducting interviews of subjects, targets, witnesses, teachers, staff,
parents, and students.
b. Serves as a liaison with any mental health providers.
c. Advises the team on school-based and community interventions and supports,
including possible mental health assessments, where appropriate.
d. Assists with next steps and possible referrals.
3. OTHER CONSIDERATIONS FOR MEMBERSHIP
a. Human Resources, (informed on personnel policy and practices).
b. Legal Counsel (as a legal advisor).
c. Ad hoc members (e.g. Special Education Case Manager).
d. Key Gatekeepers such as someone from the school (e.g., teacher, counselor,
coach, nurse, other school employee) or community (e.g., probation officer,
social service worker) who may know or have knowledge of the subject or have
special expertise to help the team deal with an issue.

7. STANDARD SAFETY AND SECURITY EQUIPMENT AND TOOLS FOR EACH CAMPUS

The list hereunder is not exclusive. It may be modified depending on the needs of each of the
Campuses of USTP.

1. Service revolvers/pistols for regular security personnel;


2. Motor vehicle for roving and patrol operations;
3. Firetruck or emergency truck;
4. Flashlights;
5. Baton;
6. Handheld radio;
7. Ambulance;
8. Close-circuit-television;
9. Metal detectors;
10. and, others.
