Re: MR Brett Collister, DOB 20/11/1970

Thank you for seeing Mr. Brett Collister, a 45-year-old man whose test result indicates diabetes mellitus
Type 2. He work as a factory supervisor and is married with three children.

MR. Collister has had been my regular patient in my practice. On 4/01/15, he presented with 4 weeks
history run down, sore eyes, tiredness and dizziness associated with orthostatic hypotension. Although
his vitals were normal, he was overweight with BMI 29.7. Hence, a blood test was ordered accordingly.
On today’s review, his test results revealed increased random glucose 13.5mmol/l, fasting glucose
7.4mmol/l, HBA1c 8.5% and HDL/LDL levels. In addition to the previous symptoms, he now complained
of decrease in his vision.

About MR Collister past medical history, he suffered from Infectious Mononucleosis in 2003. In addition,
last year he has had a rotator cuff tear injury and sore right knee, which was managed by physiotherapy
accordingly.

In view of above, I am referring this patient under your care for further assessment and management.
