I.

Demographic Information

My patient in pediatric Oncology duty at Salvador clinic is Julia Amber Gupit with the age of 14 years
old and born January 19, 2004. She lives at Pinagmukan Proper, Batangas City and she was diagnosed
with Acute Myeloid Leukemia.

II. Health Information

The following are the vital sign and taken last March 13, 2018. These are blood pressure of
100/60 mmHg, pulse rate of 95 beats per minute, respiratory rate of 22 breaths per minute and
temperature of 37.1 degree Celsius and her height is 158 centimeter and her weight was 50.05
kilograms.

According to the patient’s mother there are family health history of lung cancer, diabetes
mellitus and hypertension.

According to the patient, three days prior to hospitalization last October 26 2018, she was
experienced intermittent fever, fatigue, flu like symptoms, pale skin and muscle pain especially in her
legs. Then she went to their family doctor and gave a medication for her fever and pain reliever and
undergone for a tests such as complete blood count and with the results of low platelets and low
hemoglobin count and explained what was all about, they referred at Salvador clinic for further tests
and readings about the suspected diagnosis and the final diagnosis was Acute Myeloid Leukemia. Then
she was started chemotherapy last October 29, 2016.

The patient received a complete vaccine.

III. Nutritional Information

The patient eats three times a day with rice, chicken, fish, egg, vegetables and fruits. They
usually drink purified water but sometimes they also drink fresh fruit juices such as orange,
watermelon and etc.

IV. Activities of Daily Living

The patient sleeps for 9 to 10 hours a day as possible. According to the patient, her usual
activities in a day are reading books, drawing, making diary, doing house chores as tolerated and
bathing which she said are very important to prevent or avoid ailments since she has low immunity
then after she did something, she get rest as possible since she’s very limited to do an activities.

According to the patient, there are restrictions in activities needed to follow such as avoiding all
preservatives, or cancerous food. Avoiding strenuous activities is not tolerated and avoids crowded
places that she may acquire different infection or ailments. Rest as possible she can.

She goes to her doctor to have a checked up when she feels different or pain. But usually, she goes to
the doctor twice a week for her chemotherapy session.
Then her treatment for her condition is chemotherapy, by doing this, complete blood count
must be monitored.

There are many precautionary measures that must be follow such as, avoids crowded places,
avoid too many activities, wear a mask as possible, and avoid any kinds of injury that may lead to
infection since she has low immunity.
