AFFIDAVIT OF NON-OPERATION

            I, LOUIEVI AGNES YEE y Yap, of legal age, Filipino citizen, and

presently residing at Sagay City, Negros Occidental, after having been duly
sworn to in accordance with law, do hereby depose and say:

1. That I am the owner and sole proprietor of LOUIEVI MASSAGE &

SPA/THAI ROYALE SPA with business address at 2nd Floor, JEA
Building, Lacson St., Mandalagan, Bacolod City, Philippines;

2. That the said business stopped its operation in 2020 during the onset
of COVID 19 pandemic, until December 2021;

3. That I am executing this affidavit in order to attest to the truth of the

foregoing and for the purpose of my renewal of my business permit
for the resumption of operation of my said business.
