1. Valid permit to commence excavation works available
2. Is there proper / sufficient access and egress provided for personnel to enter into the trench?
2. Is the excavated trench freely accessible in the case of an emergency?
3. Are the access / egress free from any obstruction (i.e. materials)
4. Underground service drawing /report available?
5. Is the excavated trench excavated/sloped according to the approved submitted drawing?
6. Does the excavated trench show any physical signs of soil erosion?
8. Excavated trenches with depths above 1.5m provided with shoring? (Polyethylene sheets, canvas,
sheet piles, etc.)
9. P.E. design for shoring available for excavated trenches above 4m?
10. Is the excavated trench classified as a confined space? If yes, use Confined Space
Inspection Checklist)
11. Excavated material or superimposed loads placed away from the excavation edge to prevent it from
falling into the excavated trench or cause the banks to slip or cause the upheaval of the excavation
bed? Minimum of 2m away from the excavated edge must be achieved.
12. Adequate guard-rails provided at open edges of the excavated trench where the depth is more
than 1.5m to prevent personnel from falling into?
13. Warning signs available at the excavated trench?
14. Designated personnel available for inspection of excavated trench after inclement weather?
a. Re-submission of the excavation checklist must be made after inspection due to inclement
weather to Hexacon along with a re-endorsement of the Excavation Permit.
