REMINDERS IN DOING THE END OF SCHOOL YEAR FORMS

(F1 1ST SEM, F1 22nd Sem, 5A 1st Sem, 5A 2nd sem, 5B 2nd sem -Gr 12 only, F10, F 9)

>See to it that details to appear in each form are consistent to all


>Edit dowloaded forms from LIS for visibility purposes)
-School Year (Ex. 2024-2025)
-Font size at least 11, Times New Roman
-Margins of the cells without affecting details
-Signatories

>Details to be changed as suggested by the Checking Committee during the checking


period and are included in the Checklist Form must be changed before printing
>Print 3 copies of each form (1for D.O for the checking of diploma, 1 for the Registrar
School Record, 1 for the summary F6 to be submitted to the D. O) *Additional copy for your
IPCRF
>Ensure the signature of the signatories to appear in the form before submitting and
forwarding to the designated offices e.g. D.O, Registrar )

Economics Strand or Science, Technology Engineering, and Mathematics (STEM)

-Accomplish this with b-certificate, F137, F1
-in writing or encoding the name, BOLD LETTERS, SURNAME, FIRST NAME,
MIDDLE NAME (In full not middle initial)
-Track/Strand is in full title not initial
-is the age of the students during the enrollment
-Certificate of transfer must be signed by the teacher and the principal
-Eligibility for admission… = the next level
-Signature of parents must be up to 3rd grading
-If grades are encoded, the adviser must affix his/her initial beside the average per
semester
