Introduction- As a part of our Advance Nursing rural posting. We went to visit primary health centre.

The establishment of primary health centre in community development, helps to provide integrated health
centre to the people living in rural area. It is a historic landmark in the development of rural health
service in India. One of the main function of primary health centre is to provide maternal and child health
service.

The requirement is based on norm of one health worker (female) Auxiliary nurse midwife at
each sub centre and primary health centre. And one health worker (male) per each existing sub centre.
Each PHC serving a population of 3000 to 5000.

Background of PHC- It was established in 1983 and it was rebuilt in 04/03/1995. It comes under
National Rural Health Mission, and the programmes were running under Jananni Suraksha Yojna. From
1983 onwards it was running smoothly and then because of lack of resources it stopped function. In 1990
it started functioning again. PCH has six sub centres in different places.

Function of primary health centre in Bario-
1. Medical care- The PCH provides the regular medical treatment for the rural people. In Bario provide
in an ambulatory facility to limited number of people, often those living in a particular geographic
area. Common disease treated-
 Malaria
 Tb
 Enteric fever,
 Scabies
 Hypertension
 Eye infection
 Skin disease
 Anaemia
 Diarrhea
2. For Mother- In the primary health centre Bario they provide various facility and they help maintain
the good health. It provides the primary level and comprehensive care to the mother. The primary
health centre also conduct a investigation delivery which includes-
 Antenatal checkup for pregnant mother.
 Distribution of Iron and folic acid tablets.
 Prevention from anaemia.
 Administration of TT injection.
 Weight checking.
 Safe institution delivery.
 Provide health education.

3. For children- The primary health centre provide the child care which includes-
 Vitamin A prophylaxis.
 Provision of Iron and folic acid tablets
 Immunization
 Monthly height and weight checkup
 New born care
 Personal Hygiene
4. Control of Communicable disease- The primary health centre actively participates in the prevention
of communicable diseases. Various programme are run for the people from the prevention of
communicable disease includes-
 National tuberculosis control programme
 National malaria control programme
 National MCH programme
 National RCH programme
 National filaria control programme
5. Health education activity- The primary health centre provides health education for the promotion of
Childs health and mother health.

6. Emergency services- In primary health various emergency services are there Labour Presence of
doctor.
7. Dressing Facility- In primary health centre dressing facility are there.
8. Environmental Sanitation- The primary health centre provides environmental hygiene through the
aaya and word boy. They provide health education regarding how unclean environmental can affect
their health.
9. Jananni Suraksha Yojna- Specially care is provided to the pregnant and lactating mother. One
special Ambulance 102 is arranged for delivery.
10. RSBY- The smart card facility is available in Bario. The card is meant for five people in one family.
They provide the free treatment in people they have smart card.

Management information system-

In primary health centre there are systematic system for keeping the information and information
is very important for every institution through information they easily evaluation their work performance
in the monthly or yearly.

1. Registration Record- In registration record the mainly they registration the antenatal care, birth
registration still birth treatment.
Census Report-
 Delivery per month = 40-42
 Admission per month= 60-65
 OPD patient per = 50-60

2. Vaccination Record- In the vaccination record they record the vaccinated people of both mother
and children.BCG –Approximated 26130
Polio- Approximated – 21133
DPT - 26130
3. Treatment Record- In treatment record they keep the record of every disease which they
mention the how much people are affecting for this disease and how much people are treated
from this disease. Per day 40-50 are treated by the Doctor.
4. Death record – In death record they maintain the total death of any disease condition. They also
record according to age group like maternal death, child death older death. Death record in per
month 10-12.
5. Contraception Record- In contraception record they record how many people are use the
contraception. Like mala - D mala – N. Tubectomy, Vasactomy.

5. Known which type of patient is coning in PHC.
6. How to treatment the patient.

Conclusion- As a part of our community health nursing posting. We visited the primary health centre
at Bario. There we got an apportunity for learning more thing about that the visit.

we give Sr. Asha Lecture and Mrs. Deepika Lecture for their guidance and also there support.
