Morse Falls Scale

According to the Agency for Healthcare Research and Quality:

AHRQ (2014a), the Morse Falls Scale should be done at least once per day and with anychange in
patient status.

An important goal of the MFS is to identify WHY a patient is at risk for falls. Focusing on the areas of
riskidentified by the MFS will help to recognize specific interventions to prevent patient falls.
Face: 1 (patient has a grimace)
Legs: 0 (patient resting comfortably in bed)
Activity: 1 (patient is restless)
Cry: 0 (no crying observed)
Consolability: 2 (patient responds positively to comforting)

This tool was originally created with children for children to help them communicate about
their pain. Now the scale is used around the world with people ages 3 and older,
facilitating communication and improving assessment so pain management can be
addressed.
