CONTRACT
Families,

Science is a hands-on activity and students will be responsible to engage and complete
labs throughout the school year. Students will handle lab equipment including glass, heat
and possibly chemicals. As a school we want to ensure our students safety is put FIRST. In
order to do so we have some lab safety rules to ensure our students are safe!

Lab safety rules are to be followed at all times when completing a lab. Students and
Parents will be required to sign this contract PRIOR to students completing a lab. The
Friday before EACH lab this contract will be sent home along with a paper discussing the
lab. Students should return the permission slip no later than the following Tuesday.

Lab safety rules:

1.) Students MUST wear closed toe shoes and long pants. Students will be given lab coats
before completing the lab
2.) Long hair must be tied up
3.) Students MUST wear safety goggles during the entirety of the lab
4.) NO food or drinks in the classroom during labs
5.) Students will be required to turn in a worksheet after each lab explaining what the lab
was as well as their findings (I will give them a copy at the beginning of the lab)
6.) When students are using materials they are responsible to clean and throw away those
materials when they are done
7.) Student will use all materials carefully and respectfully
8.) Do NOT pour any liquid in the sink unless told to do so specifically
9.) Do not touch, face, eyes, hair, or skin during lab an before you wash your hands
10.) NO electronics what so ever

My child and I have thoroughly read the Lab safety contract. We fully understand what is expected
of the student and agree to follow all rules and procedures during the lab. Should the student not
listen they will be unable to participate in future labs.
