1. Accident (July 17, 2023)

a. The Safety Officer presented the Accident details:
ERT Team Head
Who: John Vincent Esmero
What: Victim passed out/lost consciousness due to inhalation of CO fumes
ELDER BANLUTA Where: While performing cleaning of echo line/mist separator of FB# 6
Fire Brigade Team When: Around 1525 hours of July 17, 2023

MARC DUTOSME Root Cause:

Evacuation Team  Victim need to undergo safety refresher training
 Lack of monitoring/safety guidance (verify compliance to SOP)

REYNALDO ALIGUAY b. Michael Ibarita added that Toolbox should be conducted before any critical

c. Randy Pedroza agreed and added that also systems are already in placed
HEAD GUARD with regards to safety precautions, it is just a matter of compliance to the said
Security Team procedures especially the management side.
d. The attendees discussed the need to purchase a gas detector, or invite a

supplier to do actual testing of the apparatus to the actual area (FB section).

2022-2023 OFFICERS e. Junard Patente will check/inquire for industrial fan at Belmont, as part of the
action plan.
f. The safety officer will present the root cause and the proposed action plan to

c. Human Resource has an incoming orientation about company policies

Harassment Seminar.
d. Junard Patente suggests that the committee will formulate Plans & Programs
ERT Team Head for the next year 2024.
e. Michael Ibarita reminds on Safety Inspection to be conducted by the
members of the committee.
ELDER BANLUTA f. Randy Abella also mentioned that it could be arranged as per section.
Fire Brigade Team g. Randy Pedroza emphasize that new members of the committee need to
undergo safety training to enhance their safety awareness needed to identify
hazards during safety inspections.
MARC DUTOSME h. The attendees agreed that safety inspections will be arranged per team and
Evacuation Team per section.
i. The safety officer will formulate a safety inspection checklist.

a. Junard Patente ask about the status for the Hard Hat stickers (as per section)
FILMAR PITOGO b. Randy Pedroza asks what sticker colors each section want.
Search & Rescue Team c. It was agreed that the safety officer will present an example, and to finalize
what stickers will be assigned to each section.
JUNARD PATENTE d. The safety officer mentioned that issue with dogs entering the plant premises
Communication Team has become a concern.
e. Dogs has been observed to scatter garbage/food wastes which became an
issue.
REYNALDO ALIGUAY f. Junard Patente suggested that he will help to try to talk with CI regarding this
Electrical Team issue.
