1. Patient was positioned supine on the operating bed

2. Patient was under general anesthesia
3. TEE probe was inserted slowly through oropharynx down to esophagus

Aorta : 3 cuspis, calcification (-). Normal Function and Movement
Mitral : Normal Function and Movement
Tricuspid : Mild Tricuspid Regurgitation (Regurgitant Jet Length < 20% RA, TR Vmax 2.77 m/s,
TR MaxPG 30.8 mmHg)
Pulmonal : Normal Function and Movement, PV Acct 114 ms, mPAP 22 mmHg, PASP 33 mmHg.

2. DIMENSION :
- LA: Normal, LA mayor 3.2 cm, LA minor 2.5 cm
- LV: Normal, LVEDd 2.7 cm, LVEDs 1.8 cm,
- RA: Dilatation, RA mayor 4.3 cm, RA minor 4.3 cm, chiari network was seen
- RV: Dilatation, RVDB 3.6 cm
3. - Normal LV systolic function, EF 60% (BIPLANE)
- Normal RV systolic function, TAPSE 2.0 cm, S’ Lateral 12 cm/s
- Regional wall motion: Global Normokinetic
4. IAS: not intact, ASD Secundum Left to Right Shunt with Defect 23 - 28 mm, with malalignment
(RIM SVC 9.1 mm, RIM IVC 9.9 mm, RIM Aorta 2 mm, no RIM Posterior, RIM Mitral 8.6 mm)
IAS size 36 mm, Qp/Qs : 2.1
5. - IVS intact
- All Pulmonary Veins Drain to LA
- PA confluens, MPA 17 mm , RPA 8.7 mm, LPA 8 mm
- Significant Dilatation of coronary sinus (Size 1.3 cm x 0.8 cm) seen near LAA view
5. TEE probe was withdrawn.

6. TEE examination was completed.

Conclusion:
- Atrial Septal Defect Secundum Left to Right Shunt with malalignment, not suitable for device
closure
- Mild Tricuspid Regurgitation with Intermediate Probability of Pulmonary Hypertension
- RA and RV Dilatation
- Coronary Sinus Dilatation, MPA dilatation
- Normal LV Systolic Function, EF 60 % (BIPLANE)
- Normal RV Systolic Function, TAPSE 2.0 cm, S’ Lateral 12 cm/s
- Global Normokinetic
