E. Offer of testimony- “The testimony of the witness is offered to

prove the following:
a. (kung unsa ang naa sa iyang JA)
b. And other relevant matters”
F. Identification of the witness of his Judicial Affidavit
a. “Do you remember that you executed an affidavit relating
to this case?”
b. “Would you be able to recognize if it is shown to you?”
c. “On the record, there is a judicial affidavit of _____, is this
the same JA that you are referring to?”
d. “On the ____ page, there is a signature on top on the
name _____, is this your signature?”
e. “Do you confirm and affirm the due execution of your JA
and that the statements you gave are the truth and
nothing but the truth?”
G. Prayer
