AFFIDAVIT OF UNDERTAKING

I, JUAN DELA CRUZ, Filipino, of legal age, single and a resident of Brgy. Gud, San
Isidro, Isabela, after having been duly sworn in accordance with law, hereby depose and
state:

1. That I recently purchased Yamaha Sniper with engine number G245255 and chassis
number no. RLCU1292 registered under my name;

2. That the above described motor vehicle was involved in an Anti-Illegal Buy-Bust
operation on August 3, 2023 at Brgy. Garit Sur, Echague, Isabela where the driver of
my motor vehicle, Alfie Asuncion, was arrested;

3. That because of the conduct of the said buy-bust operation, my motor vehicle is
currently impounded at PNP Echague;

4. That I did not consent nor has anything to do with the alleged conduct of illegal
activity of Alfie Asuncion. Neither do I use said motor vehicle in any illegal activity. I
am a law abiding citizen of the state and I do not engage in any illegal activity;

5. That I am executing this affidavit to support my request for the release of my

impounded vehicle;

6. That I hereby undertake to produce said vehicle upon request of the authorities if
such will be needed for further proceedings or investigation;

4. That I am executing this Affidavit freely and voluntary and to attest to the truth of the
foregoing facts and for whatever legal purpose or intent this may duly serve.
