AFFIDAVIT OF LOSS

I, JONEL GUEVARRA VILLALOBOS, of legal age, Filipino, single

and a resident of Damayan St., Barangay Navarro, General Trias City,
Cavite, after having been duly sworn to in accordance with law, depose
and state THAT:

1. I am a valid holder of a Driver’s License ID No. D06-10-007488

issued by Land Transportation Office in accordance with pertinent
laws, rules and regulations;

2. I usually put my driver’s license in my bag. Unfortunately, when I

tried to find it to use it as my valid ID for a Deed of Sale
transaction, I noticed my driver’s license was already gone;

3. Diligent efforts were exerted to locate said Driver’s License, but

despite my diligent search, it could not be found and the same is
now beyond recovery;

4. That the said Driver’s License had not been confiscated by the
police or other competent authority;

5. As such, I am executing this Affidavit of Loss to attest to the

truthfulness of the foregoing and to support my application for the
issuance of a renewal of Driver’s License, in lieu of the one that
was lost.

IN WITNESS WHEREOF, I have hereunto set my hand this 15th
