AFFIDAVIT OF LOSS

I, VINCE ERNEST ISAAC, of legal age, Filipino, single and with home address
at Brgy. Soyung, Echague, Isabela, under oath, hereby depose and state THAT:

1. I am an Electronics and Communication Engineer for which I was issued

an identification card by the Professional Regulation Commission;

2. Recently, when I checked my bag where I used to keep my PRC ID, it is

nowhere to be found. I just transferred from Manila to Isabela and I might have
misplaced my PRC ID unintentionally;

3. I exerted diligent effort to locate my PRC ID. I even asked my landlady at

my apartment at Manila but to no avail. Hence, my PRC ID is now declared loss;

4. I am voluntarily executing this affidavit to attest to the truth of the

foregoing for any legal purpose it may serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature this 24th day of
