AFFIDAVIT OF LOSS

I, MARK G. BILBAO, of legal age, and residing at Brgy.

Botong, Oton, Iloilo, after having sworn in accordance with law,
hereby depose and say, that:

1. That I am a graduate of Bachelor of Science Major in Marine

Engineering from John B. Lacson Maritime University, Molo, Iloilo
City;

2. That I was issued a College Diploma by said University;

3. That said Diploma was missing and must have been lost
sometime in February or March 2023 when I lost some of my
important documents contained in a brown envelope;

4. That despite long and diligent it could no longer be recovered;

5. I am executing this Affidavit to attest to the truth of the foregoing

and to support a request for a replacement copy of the above-
mentioned Diploma.
