Recognition Policy: Employee of the Month

Purpose
Altus Infusion appreciates the efforts of its employees to achieve corporate goals and to fulfill
its vision. Outstanding employees deserve to be recognized both as a reward for exceptional
performance and as a model to other employees!
Eligibility
All full-time and part-time employees are eligible to participate and potentially be selected in
the Employee of the Month Program. Employees eligible to be selected will have been
employed for at least 6 months and are not in a probationary, performance improvement plan,
or other disciplinary status, at the time of nomination. Nominees must also be approved by
their manager. An employee may only be designated as an Employee of the Month once during
a calendar year.
Criteria
The Employee of the Month Program recognizes one employee each month. Nominations for
the award (including self-nominations) may be submitted by any employee using the
nomination form available from Human Resources (HR). Nominations are based on several
criteria which can be found on the next page.
Process
Completed nomination forms must be submitted to HR between the 1st and 20th day of each
month to be considered for the following month's award. Before the end of each month, the
Employee of the Month Selection Committee will meet to review the nominations and select
the Employee of the Month. The committee will consist of 5 individuals.
Employee of the Month Selection Committee
The committee will be comprised of both management and non-management employees
selected annually by HR. Members of the committee will not be eligible to receive the
Employee of the Month award during their time of service.
Past Employee of the Month winners can be considered for appointment to the committee.
Awards
The selected Employee of the Month will be announced at the beginning of the following
month via companywide email, the company's website, and its social media accounts. As
recognition for being selected as the Employee of the Month, the employee will receive a
reserved parking space for that month, along with several other rewards. Additionally, the
selected employee will be included along with the 11 other Employees of the Month for the
current year in consideration for the Employee of the Year Award given each January.
Employees who have questions or need assistance with this policy are encouraged to contact
their immediate supervisor or HR.
Criteria
Credibility
This dimension measures how the person is perceived in terms of their communication skills,
competence, clarity and integrity. A highly credible employee is usually very dependable. They show up
on time, do what they say and say what they do. They are honest and straightforward with their
coworkers.

Respect
A respectful person is not one who always agrees with everyone, but rather someone who values the
opinions of their coworkers even when they don’t agree with them. Someone with a high level of
respect makes a constant effort not to discriminate or offend others for their decisions or lifestyle. They
are always appreciative, polite, considerate and recognize the value in other people’s points of view.

Alignment
Open and honest lines of communication. It means working every day to create a healthy and warm
work environment. The best companies to work for usually exhibit a high level of humanity that can only
be made possible when there’s a sense of camaraderie and affinity.

Continuous Improvement

Employee must demonstrate a willingness to take initiative beyond regular job assignments,
resulting in inspiring and supporting the performance and achievement of others.

Integrity

Employee must demonstrate the ability to work as a team member. Consistently dependable
and punctual in reporting for duty, completes assignments on time and has a distinguished
attendance record.

Patient Focused
People who come in to work, not because they have to, but because they are passionate about what
they do have the highest level of commitment to their job. They are always putting the patient first and
are committed to their job.
