Sir / Madam:

In behalf of our client, MIKA S. LINA, of Blk10 Lot 1, Etcang Homes,

Brgy 169, Bohol City, this is to kindly request for a copy of CCTV footage/s in
your establishment on September 23, 2020 in connection with an ATM
Skimming incident perpetrated by an unidentified person/s against said
client.

At the outset, this request stemmed from a skimming incident

committed against our client that occurred at BDO ATM machine located
along General Luis Street, Barangay Nova Proper, Novaliches, Quezon City
on September 13, 2020, specifically at 11:00AM to 1:00PM and 3:00PM to
9:00PM. Said incident was duly reported by our client to the Novaliches
Police Station (PS-4) on September 16, 2020. Attached is a copy of a
Certification of “Incident for Record” which has been the subject of an on-going
police investigation.

Immediately thereafter and on even date, said skimmer visited your

establishment and purchased mobile phones at INFONEC, Savemore
Novaliches, using our client’s LBP ATM Debit Card for payment thereto.
While our client was able to obtain copy of the CCTV footage of the suspect
transacting in said store; yet, the identity of the suspect was not established as
he was wearing a face mask and face shield the entire time. Consequently,
pertinent CCTV footages of the said suspect recorded on said date could help
in establishing the identity of the suspect.

 Notably, we were told by our client that she had long requested the
Security Head of your establishment for a copy of the said CCTV footage.
However, as of this date, said copy is yet to be provided.

In view thereof, as the identification of the Skimmer is still unknown,

the police investigation on said matter has been put on hold as the skimmer is
yet to be identified. Verily, the CCTV footage/s taken and recorded in your
premises on said date would be of great importance to establish the identity
of the said suspect. Hence, it is respectfully requested that we be provided a
copy of pertinent of said CCTV footage/s to assist in the police investigation
and for the filing of an appropriate criminal action against said Skimmer.

Your assistance and cooperation in this matter is kindly sought.
