In behalf of our client, MIKA S. LINA, of Blk-13, Lot-26, Estrella Homes

II, Brgy 169, Caloocan City, we respectfully request for a copy of the CCTV
footages with regards to an incident that occurred at BDO ATM machine
located along General Luis Street, Barangay Nova Proper, Novaliches,
Quezon City on September 13, 2020, specifically at 11:00AM to 1:00PM and
3:00PM to 9:00PM. To prove this is a copy of Certification of Police Blotter
relative to said incident.

Relative thereto, on September 22, 2020, PLt. EDONG LAT, made a

similar request to your good office for the usual investigation being
conducted on the reported ATM skimming incident against our client, as well
as a letter request from our client on September 23, 2020 for a copy of said
CCTV footage. Attached are copies of said requests. Understandably, our
client told us that you would need a period of five (5) months to retrieve the
requested footage and a copy sent to the police investigator and/or to our
client. However, almost seven (7) months have elapsed and the requested
footage has yet to be sent.

Said CCTV footage will be for our client’s personal use and for the
investigation and possible filing of a case against the responsible person /s on
the ATM Skimming incident that transpired on September 13, 2020 at said
BDO ATM machine.

In this regard, undersigned respectfully request a copy of CCTV

Footage to be reviewed from 11:00AM to 1:00PM and 3:00PM to 9:00PM of
September 13, 2020 for possible retrieval of any detailed information relative
to the said incident.
