Mr. McCarthy was admitted on 6 April 2018 with an infected right diabetic foot ulcer and
positive blood cultures. Past medical history includes benign prostatic hyperplasia, diabetes
mellitus Type 2, dementia, hypertension, and peripheral vascular disease.
A vascular surgeon was consulted, who recommended a right below the knee amputation,
which was performed without complication. Following surgery, Mr McCarthy was placed on
IV antibiotics and pain medications which have been successfully transitioned to oral admin-
istration. His wound is healing well and repeat blood cultures are negative.
Mr. McCarthy is now stable for discharge to your facility. He will continue to need assistance
for ADLs and a wheelchair for mobility. His surgical site should be assessed for infection,
with daily dressing changes. He should also receive frequent turning, to prevent pressure
ulcers.
Of note, his wife wanted him to their be discharged back to home under her care; given his
dementia and decreased mobility following the amputation, physiotherapy and occupational
therapy staff did not believe this was a safe discharge plan. After some time at your facility, he
may show sufficient improvement to return home or a decision made for long-term care place-
ment.
