PETITION FOR CORRECTION OF CLERICAL ERROR IN THE

CERTIFICATE OF LIVE BIRTH

I, PATRICIA CHUA, of legal age, single, and residing at No. 22, Connecticut Street,
Ayala Heights, Quezon City, Philippines, having been sworn in accordance with law,
depose and say that:

1. I am a petitioner seeking correction of the clerical error in my Certificate of Live

Birth;
2. I was born on May 22, 1984, at Quezon City, Metro Manila.
3. The birth was recorded under registry number 987654.
4. The clerical error’s to be corrected are:

5. The facts/reasons for filing this petition are the following:

a. The intended First or Given Name of Petitioner is “PATRICIA” and not
“Patrisia”. Petitioner grew up being known as Patricia by her own
parents and the people around her. Petitioner also used the first name
“PATRICIA” in her school records and the university and she still uses the
said first name in all her business and official transactions. The intended
first name was also the one stated in her medical records and Baptismal
Certificate
b. Under the Certificate of Live Birth issued by the Local Civil Registry of
Quezon City, the last name of the petitioner was spelled as “CUA”, thereby
omitting the letter “h”, as it should be “CHUA”. It was shown that as
reflected in the petitioner’s biological father’s certificate of live birth, the
correct spelling is “CHUA” and not “CUA”. 
6. I submit the following documents to support this petitioner:

b. Baptismal Certificate of Petitioner

e. Marriage Certificate of the Biological parents of the petitioner
f. Certificate of Live Birth of the biological father of the petitioner
7. I have not filed any similar petition and that, to the best of my knowledge, no
other similar petition is pending with any LCRO, Court or Philippine Consulate. 
8. I am filing this petition at the LCRO of Quezon City, in accordance with R.A 9048
as amended and the implementing rules and regulations.
