at Bayambang, Pangasinan. Through this participation, the students are exposed to the culture and tradition of
other places and expanded their socialization with their fellow Pangasinense.

I/We have considered the benefits that my son/daughter will derive from his/her participation in the said
activity and that the LGU Tourism Office, school, and/or DepEd employees and personnel are not held
responsible for any untoward incident that may happen beyond their control.
