The undersigned wishes to inform you that your child/ward _______________________ together with his/her classmates
will undertake an off-campus activity on ___________________________________________________ at
INC Museum (Central Ave, Q.C.), KKK Museum & El Deposito (San Juan City), Enchanted Kingdom with Agila
Theatre (Sta. Rosa, Laguna).
The purpose of this activity is to educate our students about the different applications of the concepts they learned inside
the classroom through the educational trips. The payment is Php 1,950.00.
In view of this, we wish to obtain your consent for him/her to join/attend this activity by signing the waiver and the parent
