SPECIAL POWER OF ATTORNEY

KNOW ALL MEN BY THESE PRESENTS:

I, Leila B. Cadungog, of legal age, Filipino, married, with residence and postal address at
L2 B16 Volta St., Meritville Townhomes Pulanglupa Uno Las Piñas City, do hereby name, appoint and
constitute Sylvia Guillermo Leyva as my true and lawful attorney-in-fact to act for in my name, place and
stead, and on my behalf, to do and execute any or all of the following acts, deeds, and things:

1. To apply and secure a Certified True Copy (CTC) of my Live Certificate of Birth issued by the
Local Civil Registrar of the Municipality of Tumauini, Isabela;

2. To appear before the said Local Civil Registrar in connection with the procurement of such
other documents pertaining to the application of the same, with authority to prepare or cause to
be prepare, sign, and execute any and all documents required to be submitted and filed in
connection thereto;

3. To transact, process, and arrange the said CTC of my Live Birth Certificate, including the
authority to sign, execute, process and deliver on my behalf, any requests for procurement
thereof, if any, and any and all other documents evidencing or relating to the process; and to
receive and secure any and all documents, instrument, and papers necessary and incidental for
the procurement thereof, if any, with authority to sign any and all documents, requests and other
writings in connection with the release of the same as may be necessary for its processing and
all other obligations relating to such process to or from your office; and to pay out and discharge
from any funds in his hands, belonging to me, such sums of money as may be necessary to
cover any and all fees appurtenant thereto for such application, with authority to sign, execute,
deliver and receive any and all documents, requests and all other writings in connection with
the said process and all other obligations relating to the same.

HEREBY GIVING AND GRANTING unto my attorney-in-fact full power and authority
whatsoever requisite, necessary, or proper to be done in or about the premises, as fully to all intents and
purposes as I might or could lawfully do if personally present, and hereby ratifying and confirming all that
the said attorney in-fact shall lawfully do or cause to be done by virtue of these presents.
