Procedure:

Pulse        
1. Wash hands.        
2. Identify the client.        
3. Explain the procedure to the client.        
4. Position patient either lying down or seated, with
palms up.        
5. Locate the pulse point that is easiest to reach.
Normally, this is the radial, brachial or carotid site.        
6. Palpate the pulse site using the index and middle
fingertips on pulse point.        
7. Count the pulse rate for one full minute.        
8. Observe for irregularities of the pulse.        
8. Note the reading and findings.        
Respiration        
9. Pretend that you are still taking the pulse rate,
fold the patient's arm diagonally across his chest,
allowing you to feel the movement instead.        
10. Count the number of respirations for 1 minute.
One inhalation and one exhalation is equivalent to
one respiration.        
11. Observe the respiration rate, rhythm, and
depth.        
12. Note the reading and findings.        
13. Wash hands.        
14. Document the readings (pulse rate & respiratory
rate) on the chart.        
15. Inform the physician if abnormalities in the
findings were noted.
